Study On The Incidence Of Adenoid Hypertrophy Complicated With Sinusitis In Children And Its Therapeutic Effect

Objective: To determine the distribution of sinusitis in children with adenoid hypertrophy in Dalian by analyzing large samples of children with adenoid hypertrophy in this area.Then analyze the incidence and age-related relationship between them and provide a statistical basis for the diagnosis and treatment of children's sinusitis.Methods: 991 cases of adenoid hypertrophy were examined by SCANORA3 Dx CT to determine the presence or absence of inflammation of the sinuses.And the incidence of sinusitis in different age groups and different sinus' s groups was analyzed.Among 991 cases of adenoid hypertrophy,596 children's age were below 6 years old,295 children ranged age from 6 to 9 years old and 100 children ranged age from 10 to14 years old.The inflammation in different groups of sinuses was mainly analyzed.The incidence of different subgroups was tested by using SPSS17.0 statistical software.P<0.05 is a statistically significant difference.Results: According to the incidence of sinusitis in 991 cases of adenoid hypertrophy,among 596 cases in patients who were all younger than 6 years old,the incidence of maxillary sinusitis was 56.2%,and the incidence of complicated sphenoidal sinusitis was 32.7% The incidence of ethmoid sinusitis was 46.8%,while the incidence of maxillary sinus with ethmoid sinusitis was 40.3%,the incidence of sinusitis in the whole group was 25.3%.Among 295 patients who ranged age from 6 to 9years old,the incidence of the maxillary sinusitis was 41.7%,the incidence of sphenoid sinusitis was 27.1%,the incidence of ethmoid sinusitis was 39%,while the incidence of maxillary sinus with ethmoid sinusitis was 30.8%,the incidence of the whole group of sinusitis is18.6%.According to the group of 100 patients who ranged age from 10-14 years old,the incidence of maxillary sinusitis was 30%,the incidence of sphenoid sinusitis was 9%,the incidence of patients with ethmoid sinusitis was 25%,While the incidence of maxillary sinus with ethmoid sinusitis was 16%,the incidence of sinusitis in the whole group was 8%.The statistical analysis showed that the incidence of sinusitis in all age groups was significantly different(P <0.05).Children,under the age of 6 years old,whose incidence of adenoid hypertrophy with sinusitis is the highest,but with age's ascending,the incidence showed a downward trend changing.In addition,children's adenoid hypertrophy with ethmoid sinus often combined with maxillary sinusitis.The distribution characteristicsof patients with maxillary sinusitis or ethmoid sinus,in these age levels,was consistent with the distribution of total sinusitis in Dalian.Conclusions: Among children with adenoid hypertrophy in Dalian,the incidence of sinusitis is the highest in patients under 6 years old,so that the diagnosis and treatment of them should be emphasized.The inflammation of the maxillary sinus and ethmoid sinus is most frequently associated with adenoid hypertrophy,which became the obstruction of the nostril,which could affect the drainage of ostiomeatal complex and sinus.Meanwhile,it was related to characteristics of sinus development in childhood.So the period should drew more attention of us.according to the test results,children should be especially concerned when they had stuffy nose,runny nose,headache and other related symptoms.Then imaging examination to further investigating the situation of the sinuses is needed so that it is more beneficial to prevent and treat the children with adenoid hypertrophy and sinusitis.Objective: To investigate the clinical treatment of children with adenoid hypertrophy complicated with sinusitis.Methods: SCANORA 3Dx CT was performed on 157 patients with adenoid hypertrophy for nasopharyngeal examination to determine the inflammation of the sinuses.Various treatment regimens were all performed for patients in different ages.157 cases of adenoid hypertrophy cases were divided into 3 groups,.The first group,55children's age were all below 6 years old,the second group,52 children ranged age from 6 to 9 years old,the third group,50 children ranged age from 10 to 14 years old.In each group,the application of adenoidectomy combined with drug treatment,simple drug therapy,simple Adenoidectomy treatment were analyzed separately.Using SPSS17.0 statistical software for the cure rate of different programs X 2 test.P <0.05 for the difference was statistically significant.Results: The statistical analysis showed that the cure rates of all age groups were statistically different(P <0.05)in 157 cases of adenoid hypertrophy complicated with sinusitis.The cure rate of the youngest children's group was the lowest.And with the increasing of age,the cure rate is on the rise.Meanwhile,in the same age group,the cure rate of each treatment group was statistically different(P <0.05).The cure rate of the adenoidectomy combined with medical treatment's group was the highest,while the cure rate of the simple drug treatment's group was the lowest.The cure rates in all age groups were consistent.Conclusions: Pediatric adenoid hypertrophy complicated with sinusitis is a common disease.Clinically,the two diseases are closely related,and adenoid hypertrophy and sinusitis may affect each other's condition.The resection of thehypertrophic adenoid could relieve the obstruction of nasal and nasopharyngeal cavity and remove the source of the infection.In order to restore nasal sinus mucosa ciliary motor function and immune function,continuous medication's applying is needed after the adenoidectomy.continuous medication for more than 1 month is proposed.And the changes of patients' condition should be regular clinic follow-up observed.
