Objective:To observe the differences and distribution characteristics of clinical objective indexes in patients with chronic cholecystitis of different TCM syndromes,the clinical data of 188 patients with chronic cholecystitis were collected and analyzed.To analyze the correlation between clinical objective indicators and TCM syndromes in patients with chronic cholecystitis and the TCM syndromes distribution in patients with chronic cholecystitis.Methods: A total of 188 chronic cholecystitis patients admitted to department of gastroenterology of our hospital from September 2015 to November 2017 were analyzed.All selected patients were subjected to strict TCM classification.There were79 males and 109 females,with an average age of(59.0 ± 14.9)years.According to different TCM syndromes,the patients were divided into four groups.77 in Hepatobiliary damp-heat syndrome group,59 in liver depression qi stagnation group,31 in stagnant blood stagnation group and 21 in liver yin deficiency group.All selected patients were measured height,weight,gallbladder ultrasound,gallbladder contraction function tests,liver function tests,blood lipids,fasting blood glucose and other indicators.The patients' age,sex,course of disease,BMI,ratio of diameter and longitudinal diameter of gallbladder,gallbladder wall thickness,the gallbladder contractility,?-glutamyl transferase(GGT),with gallbladder stones or not,GGT,total bilirubin(TBIL),direct bilirubin(DBIL),total cholesterol(TC),triglyceride(TG),high density lipoproteincholesterol(HDL-C),low density lipoproteincholesterol(LDL-C),fasting blood glucose(FBG)were recorded.The distributions of sex,age and course of disease among the groups were analyzed.And the indexes of BMI,the ratio of gallbladder longitudinal diameter to transverse diameter,the thickness of the gallbladder wall,the gallbladder contractility,TBIL,DBIL,TC,TG,HDL-C,LDL-C,FBG and other indicators were statistically analyzed.To analyze the similarities and differences between various indexes in different types of chronic cholecystitis and their syndromes.Results: 1.In patients with chronic cholecystitis syndrome differentiation,Hepatobiliary damp-heat syndrome is the most common,followed by liver depression qi stagnation group,liver yin deficiency group at least.2.The level of BMI,the ratio of gallbladder longitudinal diameter to transverse diameter,?-glutamyl transferase,total bilirubin,direct bilirubin,triglyceride,total cholesterol and low density lipoprotein were significantly higher in Hepatobiliary damp-heat syndrome than those in the other three groups.High density lipoprotein cholesterol was significantly lower than the other three groups.3.The average BMI in liver depression qi stagnation group was significantly higher than that in liver yin deficiency group while the average ratio of gallbladder longitudinal diameter to transverse diameter was higher than that in stagnant blood stagnation group.4.The thickness of the gallbladder wall in the stagnant blood stagnation group was significantly higher than the other three groups while the average BMI were significantly higher than those in the group of liver yin deficiency.Conclusion:Hepatobiliary damp-heat syndrome in patients with chronic cholecystitis is the most common TCM syndromes.BMI index,the ratio of gallbladder longitudinal diameter to transverse diameter,GGT,TBIL,DBIL,TG,TC,HDL-C,LDL-C and other reactive lipid metabolism and physicochemical indicators of gallbladder tension are closely related to hepatobiliary damp-heat syndrome.The above indicators may provide a reference for the clinical TCM microcosmic syndrome differentiation of chronic cholecystitis.
