Objective: To analyze the clinical and pathological features,management of salivary gland tumors,investigating the survival rate of parotid malignancies and prognostic factors.Methods: The clinical data of 1818 salivary gland tumors admitted to the First Affiliated Hospital from 2002 to 2015 were retrospectively studied.Statistics of gender,age of onset,site of good hair,pathological type,clinical symptoms,auxiliary examination results,surgical methods,and analysis.Surgical data of parotid malignant tumors were collected in SPSS software,Kaplan-Meier was used to analysis the survival rate of patients with parotid malignancies,SPSS software was application of the overall survival curve,different clinical stages,different pathological grading survival curve,logarithmic rank and Wilcoxon test were used to analysis the prognostic factors associated with overall survival of parotid malignancies.Results: 1818 cases of salivary gland tumors were treated in the First Affiliated Hospital from 2002 to 2015.There were 930 male patients and 888 female patients;the age group of 40 to 50 years old was more common,with an average age of 48.85 years.The tumors were occurred in 1423 cases of parotid gland,221 cases of submandibular gland,159 cases of small salivary gland,and and14 cases of sublingual gland.A total of 1556 benign tumors,the most common pathological type is pleomorphic adenomas(908),followed by Worthin tumors(430),and 218 cases of other benign tumors,such as basal cell adenoma,myoepithelioma,eosinophilic adenoma,adenoid cystic.Carcinoma were consisted of: 83 cases of adenoid cystic carcinoma,74 cases of mucoepidermoid carcinoma,34 cases of acinar cell carcinoma,71 cases of other malignant tumors such as epithelial-myoepithelial carcinoma,salivary gland ductal carcinoma,nonspecific adenocarcinoma,cell carcinoma,squamous,cancer in pleomorphic adenoma.Salivary gland tumors are clinically manifest as painless growth mass,the accuracy rate of fine needle aspiration cytology coincidence was 91.6%,and the rate of rapid intraoperative frozen diagnosis coincidence was 85.2%.The median follow-up period of parotid gland cancer was 30 months(range,4 to 134 months).The Kaplan-Meier calculated survival rate,5-year survival rate and 10-year survival rate were 82.8% and 72.9% respectively for patients with parotid malignancy.Log-rank test showed that distant metastasis and differentiation were the most significant clinical prognostic factors,and age,T stage,cervical lymph node metastasis and clinical stage also had statistical significance,while gender,postoperative radiotherapy Statistical differences.Wilcoxon test analysis showed that T stage,distant metastasis,clinical stage and pathological differentiation were independent prognostic factors of parotid malignancy.5-year survival rate of late clinical stage,poorly differentiated parotid malignancies patients receiving postoperative radiotherapy is higher.Conclusions: 1.Salivary gland tumors are more common oral and maxillofacial surgery tumors can occur at any age;parotid gland is the most common site of disease,followed by the submandibular gland in the small salivary gland mainly occurs in the palate;both benign and malignant tumors are the most frequent parotid gland.2.In benign tumors,pleomorphic adenoma and adenolymphoma are the most common pathological types.In malignant tumors,adenoid cystic carcinoma is the most common pathological type;B-ultrasound,CT and fine-needle aspiration are helpful for diagnosis.3.The median follow-up period of patients with parotid malignancies was 30 months(range,4 to 134 months).The 5-year survival rate and 10-year survival rate were 82.8% and 72.9%.4.Log-rank test showed that distant metastasis and pathological differentiation were the most significant influencing factors in the prognosis of parotid malignancies.Age,T stage,cervical lymph node metastasis and clinical stage were also associated with the prognosis of parotid malignant tumors.While gender,postoperative radiotherapy had no statistical difference.5.Wilcoxon test analysis showed that T stage,distant metastasis,clinical stage and pathological differentiation were independent prognostic factors of parotid malignancy.6.Patients with parotid malignancies clinical stage later,the worse the degree of tumor differentiation.5-year survival rate of late clinical stage,poorly differentiated parotid malignancies patients receiving postoperative radiotherapy is higher,these patients require surgery combined with postoperative adjuvant radiotherapy.
