Objective To observe the curative effect of acupotomy and acupuncture on pain, edema degree and upper limb movement function in patients with shoulder-hand syndrome period ?post-stroke.To evaluate the the therapeutic effect of acupotomy and acupuncture on patients with SHS period I post-stroke.To the find better treatment and to provide new ideas and technical methods for the treatment of SHS after stroke. At the same time,to observe the recovery of median nerve injury after acupotomy and acupuncture. Methods There were 51 patients with SHS period I post-stroke who were in accordance with the diagnostic criteria and inclusion criteria. The therapeutic regimens for these patients were selected based on a random number taber.51 patients were divided into treatment group(26 cases)and control group(25 cases). Both groups received basic drug therapy and functional exercise. The treatment group was given a cupotomy treatment 1 times a week,a total of 4 times. The control group was given a cupuncture treatment 6 times a week,a total of 4 times. To perform VAS score, hand swelling score,Fugl-Meyer(upper extremity)movement function score before treatment,on 1 day after treatment, after 2 weeks of treatment and after 4 weeks of treatment. To observe the therapeutic effect of cupotomy and acupuncture on the base of comparison of datas between and within the groups. At the same time, the conduction velocity of the median nerve was detected before and after the treatment, and the nerve injury of SHS was observed. All statistical data were analyzed by SPSS19.0 statistical software. Results 1.After the first day of treatment, acupotomy treatment of pain improved significantly better than acupuncture treatment,by statistical examination P<0.05,the two groups were statistically significant, indicating that acupotomy treatment of shoulder pain after stroke shoulder-hand syndrome improved with immediate effect.2.Compared with these before treatment, after 2 weeks and 4 weeks of treatment, both groups were effective in reducing the VAS and hand swelling scores, and significantly increased the FMA score by statistical examination P<0.05,indicating that acupotomy treatment and acupuncture treatment can relieve the pain, reduse edema and improve upper limb movement function.3.Compared with control group, the overall efficiency of treatment group was better, after 2 weeks and 4 weeks of treatment the VAS and hand swelling scores were lower and the FMA score were higher,by statistical examination P<0.05,indicating that acupotomy treatment was better than acupuncture treatment.4.The median nerve SCV and MCV of the patients in the two groups after 4 weeks of treatment were significantly higher than those before treatment, and the difference was statistically significant(P<0.05),indicating that acupotomy treatment and acupuncture treatment can improve the conduction velocity of the median nerve. Conclusion Acupotomy treatment and acupuncture treatment on patients with shoulder-hand syndrome period ?post-stroke can relieve the pain, reduse edema, improve upper limb movement function and improve the conduction velocity of the median nerve.Acupotomy treatment was better than acupuncture treatment.Acupotomy treatment have significant immediate effect in the respect of alleviating pain.
