Objective: to observe the relationship between serum Vaspin,Chemerin and Omentin levels of Obstructive sleep apnea OSA in patients with OSA and insulin resistance,and to explore the role of three fatty factors in the pathogenesis of OSA.Methods: 65 cases of OSA patients who were admitted to our hospital from July 2016 to October 2017 were collected.The clinical data of 30 healthy control groups were selected during the same period,and the Polysomnography(PSG)was examined,and the severity of the disease was divided into three groups: the healthy control group,the light and moderate group(ahi5-30 times/hour),and the severe group(AHI >30 times/hour).Keep record of BMI,waist circumference,neck circumference and blood pressure.Fasting venous blood was pumping test Fasting glucose,Fasting blood glucose FPG),blood lipid level of each index,the application method of double antibody sandwich enzyme-linked immunosorbent(ELISA)to detect serum Vaspin,Chemerin,Omentin,Fasting insulin,Fasting insulin FINS)level and insulin resistance index,application of quantitative RT-PCR test three visceral fat factor mRNA expression level,Using Pearson and Spearman to analyze the relationship between fat factors and various indicators;Stepwise regression analysis the influencing factors of three fat factor.Results: the expression of Vaspin and Chemerin in the group was higher than that in the control group(P < 0.05),and the expression of Vaspin and Chemerin in the severe group was higher than that in the control group(P < 0.05).1.2 the expression of Omentin was lower than that in the control group(P < 0.05),and the expression of Omentin in the severe group was lower than that in the moderate group(P < 0.05).2.1 correlation analysis results show that Vaspin levels and Chemerin,waistline,neck circumference,BMI,AHI,ESS,FBS,FINS,IR,TG,TC and LDL were positively correlated(r were 0.553,0.584,0.739,0.697,0.496,0.279,0.347,0.415,0.349,0.506,0.475,0.506,P < 0.05).It was negatively correlated with Omentin,LSao2 and HDL(r was-0.603,-0.482,-0.572,P < 0.05).Stepstep regressionanalysis showed that cervical circumference,Neck circumference?BMI,AHI?HDL and DBP were the influencing factors of serum vaspin level in OSA patients(0.07,0.103,0.009,-0.682,-0.009,P < 0.05).2.2 Chemerin level and waist,neck circumference,BMI,AHI,FBS,FINS,IR,ESS,TG,TC and LDL were positively correlated(r were 0.29,0.487,0.602,0.553,0.333,0.389,0.474,0.358,0.433,0.429,0.433,P < 0.05),and negatively correlated with Omentin,LSao2,HDL(r-0.579,0.518 and 0.371 respectively,P < 0.05).BMI and AHI were the influencing factors of serum chemerin level in OSA patients(5.422,0.539,P < 0.05).2.3 Omentin and waist,neck circumference?BMI?AHI?ESS?FBS?FINS?HOMA-IR?TG?TC?LDL showed a negative correlation(r)respectively 0.399,0.528,0.541,0.617,0.412,0.342,0.486,0.467,0.517,0.374,0.317,P < 0.05),and positively correlated with LSao2,HDL(r)respectively 0.603,0.312,P < 0.05).The gradual regression analysis showed that AHI,BMI,TG and ESS score were the influencing factors of serum Omentin level in OSA patients(-0.159,-1.084,-5.815,-0.255,P < 0.05).2.4 FINS?FBS and BMI were the influencing factors of IR-HOMA in OSA patients(0.228,0.746,0.017,P < 0.05).3 RT-PCR 3 fat factor m RNA change trend was consistent with the change trend of serum expression level.Conclusion: 1,the serum level of Vaspin and Chemerin in patients with OSA increased,the level of Omentin decreased,the expression of mRNA in Vaspin and Chemerin blood cells increased,and the mRNA expression of Omentin decreased,which was related to the severity of the disease.2,Vaspin,Chemerin and Omentin are closely related to OSA insulin resistance.
