Objective:Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a chronic disorder characterized by episodic collapse of the upper airway accompanied by cessation (apnea) or reduction (hypopnea) of airflow that results in intermittent hypoxia and fragmented sleep. In this article,we will study the relationship between OSAHS and carotid atherosclerosis, insulin-resistance. Compared with opposit sex,age,BMI,TC,the conditions showing without differences between two gender,carotid IMT and HOMA-IR are compared respectively between in the OSAHS group and in the non-OSAHS group. The correlations of AHI,total time of hypoventilation and apnea,LSpO2 and carotid IMT were analyzed. Then the correlation between AHI,total time of hypoventilation and apnea,LSpO2 and HOMA-IR were also analyzed.Methods:Submitted in our hospital From december 2008 to december 2009,31 objects(OSAHS) of research have detected by polysomnography recorder detect more than 5 times of sleeping snoring or apnea or daytime sleepness conditions within an hour. Cross reference 31 objects(non-OSAHS) in the same period in our hospital, age, sex, BMI, BP have matched, less the 5 times AHI per hour. All objects with patients history(include chronic history and life style), measured height, weight, Carotid Doppler ultrasound measurement of carotid artery, IMT, fasting blood sample. The blood specimens for fasting plasma glucose (FBP), Fasting insulin levels (FINS), total cholesterol(TC) sample.2 groups without under conditions like, Myocardial infarction, heart failure, stroke, renal failure. Compared 2 groups HOMA-IR and carotid IMT. Compared with HOMA-IR and carotid IMT and sleep apnea index, hypopnea and apnea of total time, late-night minimum SpO2 and average SpO2 for Spearman correlation testing, r≠0, and P<0.05 have statistics meaning. Correlations with sleep breathing disorder degree, and atherosclerosis, and insulin resistance.Results:two groups of object, sex, age, BMI, TC distribution without apparently differences(P>0.05). compared with normal group, OSAHS group's HOMA-IR and carotid IMT apparently higher. The index in the comparison between the two groups are significant differences in statistics(average P<0.05). spearman correlated analyze showing carotid IMT, AHI, hypoventilation, and total time of apnea is interrelated. With night minimum SpO2 is negative related. The HOMA-IR and AHI, hypoventilation and total time of apnea is interrelated, with night minimum SpO2 is negative related.Conclusions:OSAHS group's carotid intima-media thickness (IMT) and IRI (HOMA-IR) is significantly higher than the control group, showing OSAHS is a risk factor to cause insulin resistance and arteriosclerosis. And the thickness of the IMT, insulin resistance, AHI, hypoventilation and total time of apnea is interrelated, with night minimum SpO2 is negative related. Showing arteriosclerosis, abnormality of metabolism and OSAHS is closely related. Alert to the clinicians:for the treatment of OSAHS, and improve night hypoxia will significantly decrease incidence and mortality of cerebro-vascular disease.
