Clinical Study On Ginger-partitioned Moxibustion On The Rehabilitation Efficacy Of The Function Of Upper Limb In Breast Cancer Patients After Modified Radical Mastectomy

Objective:To evaluate the effect of ginger-partitioned moxibustion on the rehabilitation efficacy of the function of upper limb in breast cancer patients after modified radical mastectomy(MRM).With invention of ginger-partitioned moxibustion,to promote functional rehabilitation of affected shoulder joint and grip strength of the injured side,and to lower the rate of affected side of upper limb edema and the incidence of subcutaneous hydrops and to improve the quality of life of breast cancer patients after MRM.Besides,we hope that this study can provides clinical nurses with a new idea to improve the functional rehabilitation of affected upper limb in breast cancer patients after radical operation.Methods:This research adopted quasi-experimental research design.From October 2016 to August 2017,we selected 101 hospitalized patients with primary breast cancer who were initially surgical treatment in the department ofgalactophore of Tumor Hospital of the Guangxi Medical University by convenience sampling.We divided patients into two groups according to different wards of breast surgery,and the first ward of breast surgery as control group(n=52),the second ward of breast surgery as experimental group(n=49).Two groups of patients in gender,age,course,education level,occupation,surgery site,surgery time,pathological diagnosis and clinical stages were not statistically significant(P>0.05),there is comparison between control group and experimental group.After operation the patients received the conventional therapy and nursing in the control group,while there in the experimental group on the basic of the routine treatment and nursing received ginger-partitioned moxibustion in the 14 th day after modified radical mastectomy.We choose five acupoints in the operative upper limb as acupoint prescription,which is Hegu(LI4),Waiguan(TE5),Quchi(LI11),Binao(LI14)and Jianyu(LI15)and per treatment time for 40 min,twice a day,a total of 14 days.After treatment,observed and evaluated the rehabilitation efficacy of both group cases,including(1)the range of motion of shoulder of affected upper limb at preoperative and 14 th day after operation;(2)grip strength and the grip strength values of body mass index of affected upper limb at preoperative and14 th day after operation;(3)the incidence of affected upper limb lymphedema at preoperative and 14 th day after operation;(4)the rate of seroma formation after operation;(5)the scores of Quality of life Questionnaire-Breast Cancer Module23 at preoperative and 14 th day after operation.And then,we built a database for recording data and used the SPSS17.0 statistical software to analyzed the data.It is mean that the result has statistically significant when P<0.05.Results:1.Comparison of the rang of motion of shoulder in bothgroup:(1)Preoperative 1d,the rang of motion of shoulder joint movement of anterior flexion,back extension,abduction,adduction,adduction and internal rotation,adduction and extorsion,abduction and internal rotation,abduction and external rotation,horizontal abduction and horizontal adduction were no statistically significant difference(P > 0.05).(2)The degrees of freedom of shoulder joint movement of anterior flexion,abduction,abduction and external rotation,abduction and internal rotation,horizontal abduction and horizontal adduction of the experimental group significantly higher than control group at14 th day after operation(P<0.05).2.Comparison of the grip strength and the grip strength values of body mass index between two groups of participants:(1)One day before operation,grip strength and the grip strength values of body mass index showed no statistically difference between two groups(P>0.05).(2)The grip strength and grip strength values of body mass index of experimental group were higher than the control group at 14 th day after operation(P<0.05).3.Comparison of the incidence of upper limb lymphedema between experimental group and control group:There was no one had preoperative upper limb lymphedema in both group(P>0.05).The incidence of upper limb lymphedema with the experimental group was lower than the control group at 14 th day after operation,there is significantly statistical differences(P<0.05).4.Comparison of the rate of subcutaneous liquid accumulation after operation between both group:the rate of subcutaneous liquid accumulation of experimental group was lower than the control group,the differences were significantly significant(P<0.05).5.Comparison of the scores of QLQ-BR23 between two groups:(1)Before treatment,the quality of life were not significant differences(P > 0.05)between experimental group and control group.(2)After intervention,the scores of three symptoms dimensions of experimental group inarm symptoms,breast symptoms and systemic therapy side effects were significantly lighted than those of the control group,the differences were statistically significant(P < 0.05).And the scores of function dimension of experimental group in future perspective was significantly higher than the control group,the difference was statistically significant(P<0.05).Conclusion:1.The motion disorder of shoulder joints and the reduction of muscle strength with different degree are common sign of breast cancer patients after mastectomy.The ginger-partitioned moxibustion was helpful to promote functional rehabilitation of affected shoulder joint and grip strength of the injured side in breast cancer patients after operation.2.Influenced by many factors,the patients with breast cancer might develops BCRL and subcutaneous hydrops after modified radical mastectomy,but with the ginger-partitioned moxibustion can effectively reduce the rate of BCRL and subcutaneous hydrops.3.The ginger-partitioned moxibustion can greatly promoted the postoperative recovery of upper limb function and reduced the incidence of BCRL and subcutaneous hydrops in patients with breast cancer after mastectomy.After the intervention of ginger-partitioned moxibustion,it does significantly improve breast cancer patients' quality of life.
