| 1 Objective(1)To demonstrate the clinical curative effect of Liangxue Siwu Decoction on psoriasis vulgaris(blood-heat type),and prove the effectiveness of Liangxue Siwu Decoction in treating this disease.(2)To detect the leves of CD4~+CD25~+Treg and sIL-2R in the plasma of patients,and further investigate the pathogenesis status of CD4~+CD25~+Treg and sIL-2R in psoriasis vulgaris(blood-heat type).To provide some immunological basis for the treatment of psoriasis vulgaris(blood-heat type)with Liangxue Siwu Decoction.2 Method60 patients with psoriasis vulgaris(blood-heat type)were selected from the Department of Dermatology,the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine.They were randomly divided into two groups,of which30 were treated with Liangxue Siwu Decoction in the observation group and 30 in the control group with compound Zeqi granules.They were both treated for eight weeks.In addition,15 normal subjects from the health examination center were taken as healthy control group.This study was recorded twice before and after treatment,including basic information of participants,lesion area,erythema,scales,infiltration,clinical efficacy,sleep,mental status,pruritus degree and adverse reactions,etc.The expression of CD4~+CD25~+Treg in plasma of patients were detected by flow cytometry.The expression of sIL-2R in plasma of patients were measured by double antibodies sandwich enzyme linked immunosorbent assay(ELISA).3 Result(1)The observation group and control group completed 30 cases respectively.The number of termination in the observation group was five,but in the control group was three.Before the treatment,The observation group compared with control group in the basic information,skin damage condition,pruritus score and TCM syndrome score,CD4~+CD25~+Treg,sIL-2R,the differences were not significant(P>0.05).(2)The observation group and the control group after treatment respectively compared with the groups before treatment in patients with psoriasis area and severity index score,pruritus score and TCM syndrome score all significantly decreased(P<0.05).(3)Clinical Effect was no significant difference between the observation group(the total effective rate was 90.00%)and the control group(86.67%)(P>0.05).(4)The levels of CD4~+CD25~+Treg in the observation group and the control group after treatment were significantly higher than those before treatment(P<0.05),but the levels of sIL-2R in both groups were lower than those before treatment.The CD4~+CD25~+Treg levels in the plasma in the observation group and the control group were respectively higher than those before treatment(P<0.05),and the levels of sIL-2R were respectively lower than those before treatment(P<0.05).The results showed that Liangxue Siwu Decoction and Compound Zeqi Granule could both increase the expression of CD4~+CD25~+Treg in plasma of patients and reduce the expression of sIL-2R.After treatment,compared with the healthy control group,the levels of CD4~+CD25~+Treg,sIL-2R in plasma with two groups did not return to normal levels(P<0.05).(5)The incidence of adverse reactions in the observation group was 10.00%,including two cases of vomiting and one case of impaired hepatic function(alanine aminotransferase 74 U/L).The incidence of adverse reactions in the control group was6.67%,of which one case of dizziness and one case of impaired liver function(aspartate aminotransferase 53 U/L,alanine aminotransferase 61 U/L).There was no significant difference in adverse reactions between the two groups(P>0.05).4 Conclusion(1)The clinical efficacy of Liangxue Siwu Decoction in the treatment of psoriasis vulgaris(blood-heat type)is positive.And it is worth extending in clinic.(2)The levels of CD4~+CD25~+Treg in plasma on patients with psoriasis vulgaris(blood-heat type)were low,but the expression of sIL-2R was high.After treatment with Liangxue Siwu Decoction,the expression of CD4~+CD25~+Treg increased,the leves of sIL-2R decreased. |
