Objective: According to the the distribution of apocrine glands in Han people's,to explore the optimal operation of axillary osmidrosis ,by which complete removal of apocrine glands and maintation of the axillary contour and function are both achieved.Method: From September 2007 to October 2009, 87 Han people with axillary osmidrosis were treated by suction curettage and trimming of apocrine glands under the tumescent anesthesia via axillary plica incision. To define the distribution of the apocrine glands, different sites of skin and subcutaneous tissue in axilla were taken for histological examination in 20 out of the 87 patients. Result: The apocrine glands are mainly located in superficid subcutaneous fat and deep dermis, and distributed similarly to and beyond 1.0 centimeter the extent of hair follicle in Han people. Follow-up survey in 3-12 months shows that the 87 patients who were treated by suction curettage and trimming of apocrine glands under tumescent anesthesia via axillary plica incision satisfying curative effct in all cases. The clinical effective rates was 100 %. Complications occurred in 7 patients (8.0%),3 cases of haematoma(3.4%), 2 cases of superficial epidermis necrosis(2.3%),2 cases of delayed incision healed(2.3%);Recurrence of malodour was observed in 1 patients (1.1%) 12 months past operative. Conclusion: The apocrine glands are mainly located in superficial subcutaneous fat and deep dermis, and distributed similar to and beyond 1.0 centimeter the extent of hair follicle in Han people. Suction curettage and trimming of apocrine glands under tumescent anaesthesia is an excellent method for the axillary osmidrosis .By which the satisfying curative rate, and little recurrence of malodour were achieved without visible scars and limitation of upper limbs mobilities.
