Objective:1?Evaluating the psychological status of the elderly patients with type 2diabetes.2 ? Analyzed the influence factors of psychological status in elderly patients with type 2 diabetes from social demographic factors and disease factors.3 ? Assessment of self-management ability in elderly patients with type 2 diabetes,including diet,exercise,blood sugar monitoring,etc.4?Analysis of the relationship between self-management and psychological status in elderly patients with type 2 diabetes.Methods:To screen eligible elder patients with type 2 diabetes,consulting their doctors during October 2015 to April 2016.Social Demographic Characteristics Questionnaire,Disease Related Information,Self-rating Depression Scale(SDS),Self-rating Anxiety Scale(SAS),Diabetes Self-efficacy Scale(DSES),Diabetes Self-care Scale(DSCS)were used to cross-sectional study,data were extracted by the lead reviewer,cross-checked by the second reviewer,entered into SPSS 17.0 version.Research content mainly includes the patient's social demographic characteristics(age,gender,marital status,etc.),disease relatedinformation(course,glycosylated hemoglobin,major chronic complications,etc.),psychological status(depression,anxiety and self management efficiency)and self management ability(diet,exercise and blood sugar monitoring,etc.).Results:1?The psychological status of elder patients with type 2diabetes.The total score of depression status was 50.53±8.85,108 patients' the scores of SDS greater than or equal to 53 points,depression detection rate was33.5%.The total score of anxiety status was 43.29±8.62.65 patients' the scores of SAS greater than or equal to 50 points,anxiety detection rate was 20.2%.And the total score of self-efficacy was 59.20±9.79.2? The relevant factors of psychological status in elderly patients with type 2diabetes.Single-factor analysis showed that gender,educational level,family average per capita monthly income,medical payment forms,consciously economic conditions and the number of complications were influence factors for depression in elderly patients with type 2 diabetes(P< 0.05).And gender,educational level,family average per capita monthly income,medical payment forms,consciously economic conditions,the last fasting blood glucose,the last postprandial 2h blood glucose and the number of complications were influence factors for anxiety in elderly patients with type 2 diabetes(P<0.05).And gender,educational level,family average per capita monthly income and thenumber of complications were influence factors for self-efficacy in elderly patients with type 2 diabetes(P < 0.05).The statistically significant factors that were detected by Single-factor analysis were in multivariate linear regression analysis,and it showed that educational level and gender have an effect on depression in elderly patients with type2 diabetes(F=7.567,P=0.001),multiple linear regression equation for Y=50.536+1.242*gender-2.179* educational level.Gender,the number of complications,the latest postprandial 2h blood glucose and consciously economic conditions have influence to the anxiety in patients with type 2 diabetes(F=7.065,P=0.000),multiple linear regression equation for Y=43.286+1.705* gender +1.822* the number of complications +1.520* the latest postprandial 2h blood glucose +1.532*the consciously economic conditions.The number of complications and educational level have an effect on self-efficacy in elderly patients with type 2 diabetes(F=17.052,P=0.000),multiple linear regression equation for Y=59.200-3.265* the number of complications+2.542* the educational level.3?The self-management status of elder patients with type 2 diabetes.The total score of self-management of the patient was75.09±10.31.the patients have good management were just 99 cases(30.7%).After each dimension standardized,the scores of adherence to medical regimens was 81.57±16.36 points,the scores of foot caring was76.69±10.86 points,the scores of exception handling was 75.67±12.04 points,the scores of regular exercise was 75.43±15.33 points,the scores of control diet was 72.46±15.85 points and blood glucose monitoring was71.67±12.74 points.4?The relationship between self-management and psychological status in elderly patients with type 2diabetes.Self-management was negative correlated with depression and anxiety in elder patients with type 2 diabetes(r=-0.475,P<0.001;r=-0.460,P<0.001),and Self-management was positive correlated with self-efficacy in elder patients with type 2 diabetes(r=0.717,P<0.001).Discussions:1?Elderly patients with type 2 diabetes have different levels of depression and anxiety,self-efficacy also need to improve.2?Influence factors of depression in elderly patients with type 2 diabetes have included gender and educational level,influence factors of anxiety have included gender,and number of complications,the latest postprandial 2h blood glucose and economic conditions,influence factors of self-efficacy have included educational level and the number of complications.3?Self-management ability is in the medium level in elderly patients with type 2 diabetes.4?Self-management was correlated with psychological status in elder patients with type 2 diabetes,it indicated that improve patients' self-management ability form the psychological perspectiver.In the clinical work,that emphatically observing the elderly patients of female,poor financial situation,low educational level,having adversehealth behaviors and more complications is to early discover and control the development of their unhealthy emotions and provide psychological guidance timely,alleviate psychological mood disorders,improve their self-management ability.Thus it can improve quality of life and provide safeguard of reducing the risk of death.
