ObjectiveTo understand and evaluate the current situation of anxiety,depression and self-management behavior of type 2 diabetic patients,and to explore the correlation,To provide scientific basis for targeted psychological intervention;To explore application values of Stress inoculation training among patients with type 2 diabetes.Methods1.The investigation of the situation of Anxiety,depression,and self-management behaviors in patients with type 2 diabetes: The general situation of the patients was investigated by using the self-made questionnaire.The self rating anxiety scale,self rating depression scale and self management behavior scale were used to investigate the anxiety,depression and self management behavior and analyze its correlation.2.The effect of stress inoculation training on improving patients' psychological health,self-management behavior and blood glucose control: Seventy diabetic patients were randomly assigned to either case or control group.The case and control groups received the same treatment and health education,the only difference was stress inoculation training administered in the case group,which was composed of eight 60 min sessions over 8 weeks.Before and after intervention,the two groups were evaluated the anxiety,depression and self-management behavior,the blood glucose and glycosylated hemoglobin.Statistical methods include: test,chi-square test,pearson correlation analysis.Results1.The status of anxiety,depression,and self-management behavior of inpatients with type 2 diabetes.(1)Anxiety and depression:This study of 210 patients with type 2 diabetes,the total score of anxiety was(44.28±7.71),the total score of depression was(43.34±10.14).All scores were higher than the norm,with statistically significant differences in patients with type 2 diabetes(P<0.05).(2)Self-management behavior status:his study of 210 patients with type 2diabetes,the total score of self-management behavior was(80.27±14.98),in the medium level.Prescribed medication had the highest score,followed by foot care,management of high or low blood sugar and rule diet control and exercise,and blood glucose monitoring had lowest score.(3)The relationship between anxiety,depression and self-management of patients with type 2 diabetes.We found negative correlations between anxiety,depression and self-management.(r values were-0.437 and-0.346;P<0.05).2.the effect of Stress inoculation training to anxiety,depression,self management behavior and blood sugar control.(1)Before the intervention,the difference of the score of anxiety,depression and self-management behavior was not statistically significant,P>0.05.After the intervention,the score of the case group SAS was(49.59±4.78),the score of SDS was(51.67±5.75),Scores were lower than control group SAS(55.65±5.84),SDS(56.63±5.34).The score of2-DSCS was(68.10±8.26),and the score was higher the control group(62.59±7.68),and the difference was statistically significant,P<0.05.(2)Before the intervention,the difference of the score of Fasting plasma glucose(FPG)and HbA1c% was not statistically significant,P>0.05.After the intervention,compared with control group,the case group of FPG(7.38±1.46)was significantly lower than the control group(8.57±1.45),and the difference was statistically significant,P<0.05.The score of HbA1c(8.62± 0.76)in the case group was not significantly different from the control group(8.73±0.54),P>0.05.Conclusions1.The level and prevalence rates of anxiety and depression are high,and the level of self-management behavior is low in the patients with type 2 diabetes.2.Self-management behavior was negatively correlated to both anxiety and depression in patients with type 2 diabetes.3.Stress inoculation training can effectively improve the mental health status of the patients with type 2 diabetes,to improve the patient's level of self management and blood glocose control.
