Objective: Persistent ectopic pregnancy(PEP)is a common complication of conservative treatment of ectopic pregnancy,with a higher incidence of laparoscopic surgery.The main clinical manifestations are,after conservative surgery for ectopic pregnancy,serum ?-HCG decreased delay,do not fall back,if not promptly remedial treatment,may occur abdominal pain,intra-abdominal hemorrhage,hemorrhagic shock and other serious consequences.The aim of this study was to analyze the risk factors associated with the etiology of persistent ectopic pregnancy after laparoscopic surgery,and to explore the relevant prevention and treatment methods,reduce the incidence and improve the prognosis of patients.Methods: A total of 238 cases of laparoscopic conservative treatment of oviduct pregnancy in the Affiliated Hospital of XXX from 2010 to 2016 were analyzed retrospectively.Among them,18 cases were postoperative PEP.18 patients with PEP were treated with group A(study group)and 220 patients without PEP were treated as group B(control group).The changes of serum ?-HCG,the number of menstrual cycles,the size of the block,the way of operation,pelvic adhesions,resection of corpus luteum and ovariectomy were performed in the two groups.The chi-square test and Logistic retrospective analysis were used to analyze the influencing factors of PEP.Results: In the analysis of variance,the occurrence of PEP was related to the time of menstruation,the diameter of the mass,the level of ?-HCG before operation and the visible villi(P <0.05).The age,age,pelvic adhesions,Pregnancy,pregnancy is not the PEP influencing factors(P> 0.05).Intraoperative residual cavity injection of MTXor resection of pregnancy can reduce the incidence of PEP(<0.05),the surgical approach and PEP.Logistic regression analysis showed that MTX was the main risk factor of PEP(P <0.05).The effect of MTX on the use of MTX was the most significant(OR = 7.743)TheConclusion: Preoperativ?-HCG level,menopause time,mass diameter,surgical approach and other factors are the risk factors of PEP,intraoperative resection of pregnancy luteal or fallopian tube cavity injection of MTX can reduce the incidence of postoperative PEP.For ectopic pregnancy patients,preoperative,intraoperative and postoperative risk assessment of PEP is particularly important for early diagnosis and timely prevention and treatment.
