Study On Relationship Between TCM Syndrome Of Chronic Atrophic Gastritis With The Difference Of Gastric Mucosa DNA Methylation Level

Objective By observing the methylation levels of oncogenes,tumor suppressor genes as well as Mismatch repair genes in different TCM syndromes to find out the possible causes of the differences between them,provide theoretical basis for the early Chinese medical and herbal intervention to reverse DNA methylation and to prevent the worsening of diseases.Method(1)Design a questionnaire on TCM syndromes according to the literature and the first edition of the Guidelines for Clinical Study of New Drugs on Chinese Medicines;(2)180 patients diagnosed by endoscopy with chronic atrophic gastritis were chosen based on the inclusion and exclusion criteria for TCM syndrome differentiation;(3)C-14 Helicobacter Pylori(HP)test;(4)HE stain,diagnostic electron microscopy and pathologic grading by a same senior pathologist;(5)Pyrophosphoric acid sequencing on 72 patients is used to detect the c-myc,P16&hMLH1 DNA methylation;(6)Statistical analysis is done to determine the methylation levels of oncogenes,tumor suppressor genes as well as Mismatch repair genes and the correlation between them;to determine the correlation between different TCM syndromes of chronic atrophic gastritis and the Hp infection;to determine the correlation between different TCM syndromes and the pathologic grades.Result(1)The c-myc DNA methylation rates in different TCM syndromes of chronic atrophic gastritis showed statistically significant differences(P<0.01),and the c-myc DNA methylation rates in the syndromes are stomach heat impairing yin>stasis toxin stagnation>phlegm-dampness coagulation>deficiency-cold in spleen and stomach>deficiency of qi and blood>liver-stomach disharmony.The differences between the phlegm-dampness coagulation group,the stomach heat impairing yin group,the stasis toxin stagnation group and the liver-stomach disharmony group,between the stomach heat impairing yin group and the stasis toxin stagnation group and the deficiency-cold in spleen and stomach group,and between the phlegm-dampness coagulation group and the stasis toxin stagnation group and the deficiency of qi and blood group are statistically significant(P<0.05).(2)The p16 DNA methylation rates in different TCM syndromes of chronic atrophic gastritis showed statistically significant differences(P<0.05),and the p16 DNA methylation rates in the syndromes are liver-stomach disharmony>deficiency-cold in spleen and stomach>deficiency of qi and blood>phlegm-dampness coagulation>stomach heat impairing yin>stasis toxin stagnation.The differences between the stomach heat impairing yin group and the stasis toxin stagnation group and the liver-stomach disharmony group are statistically significant(P<0.05).(3)The hMLH1 DNA methylation rates are not significantly different between the TCM syndromes of chronic atrophic gastritis(P>0.05).(4)Based on each TCM syndrome of chronic atrophic gastritis,the difference between DNA methylation rates and Hp infection are of no statistical significance(P>0.05).The differences in Hp infection between the different TCM syndromes of chronic atrophic gastritis is statistically significant(P<0.05).The Hp infection severity in the syndromes are liver-stomach disharmony>phlegm-dampness coagulation>stasis toxin stagnation>stomach heat impairing yin>deficiency-cold in spleen and stomach>deficiency of qi and blood.(5)Based on each TCM syndrome of chronic atrophic gastritis,the difference between DNA methylation rates and the pathologic grades are of no tatistical significance(P>0.05).The pathologic grades of different TCM syndromes of chronic atrophic gastritis are of no statistically significant difference(P>0.05).Conclusions(1)C-myc and p16 DNA methylation are showed statistically significant differences in different TCM syndromes of chronic atrophic gastritis.(2)The hMLH1 DNA methylation rates are not significantly different between the TCM syndromes of chronic atrophic gastritis.(3)The differences in Hp infection between the different TCM syndromes of chronic atrophic gastritis is statistically significant.Based on each TCM syndrome of chronic atrophic gastritis,the difference between DNA methylation rates and Hp infection are of no statistical significance.(4)The pathologic grades of different TCM syndromes of chronic atrophic gastritis are of no statistically significant difference.And based on each TCM syndrome of chronic atrophic gastritis,the difference between DNA methylation rates and the pathologic grades are of no tatistical significance.
