Explore The Correlation Of Chronic Atrophic Gastritis Infected With Helicobacter Pylori Between Pathological Changes Of Gastric Mucosa And "Qi-deficiency""Blood Stasis"Syndrome Element In Middle-aged And Elderly Patients

Objective:Most scholars believe that the pathogenesis of chronic atrophic gastritis in the middle-aged and elderly is spleen stomach Qi deficiency,and the key to pathological progress is blood stasis.In addition,gastric mucosa pathology is an important means to diagnose and judge the development of disease,so this study explored the correlation between different types of gastric mucosa pathology and "Qi deficiency,blood stasis",to understand its internal relationship and evolution,in order to provide reference for clinical syndrome differentiation and prescription.Methods:240 patients with chronic atrophic gastritis were collected from February 2020 to February 2021 in the department of spleen and stomach of Fuzhou hospital of traditional Chinese medicine.They were divided into Hp infection group and non Hp infection group by Hp detection.The four diagnosis data and pathological results of patients were collected in the form of questionnaire survey,and the four diagnosis data were recorded into the "health status identification system of traditional chinese medicine" to obtain the syndrome element information.The database was established and analyzed by spss22.0.Results:1.There was no significant difference in gender and age between Hp group and non Hp group(P>0.05).2.There was no difference in the grade distribution of different pathological types between Hp group and non Hp group(P>0.05).3.In the distribution of syndrome elements,Hp group and non Hp group had statistically significant differences in the distribution of syndrome elements of "Qi deficiency,dampness,blood stasis,Yang deficiency,phlegm and heat"(P<0.05),indicating that Hp infection may aggravate the severity of the above disease syndrome elements.There was significant difference in the level distribution of syndrome elements of "spleen" in disease location(P<0.05),indicating that Hp infection may aggravate spleen injury.4.Because Hp group had atrophic and active inflammatory pathological changes,and the number of intraepithelial neoplasia pathological changes collected in this study was less,only the data between Hp group atrophic with or without chronic inflammation,atrophic with or without intestinal metaplasia were analyzed.There was no significant difference in gender and age distribution between Hp group with or without chronic inflammation,atrophy with or without intestinal metaplasia(P>0.05).5.There was no difference in the distribution of pathological types(active inflammation,intestinal metaplasia,intraepithelial neoplasia)between Hp group with or without chronic inflammation(P>0.05);There was no difference in the distribution of pathological types(chronic inflammation,active inflammation,intraepithelial neoplasia)between Hp group with or without intestinal metaplasia(P>0.05).6.There was no significant difference in the degree of Qi deficiency and blood stasis between Hp group with or without chronic inflammation(P>0.05);There was no difference in the distribution of Qi deficiency degree between Hp group with atrophy and without intestinal metaplasia(P>0.05),but there was difference in the distribution of blood stasis degree(P<0.05),indicating that atrophy with intestinal metaplasia was related to the degree of blood stasis.7.There was no significant correlation between atrophy with or without chronic inflammation and Qi deficiency and blood stasis syndrome elements in Hp group(OR<1,P>0.05).There was no significant correlation between atrophy with or without intestinal metaplasia and Qi deficiency(OR>1,P>0.05),but significant correlation between atrophy with or without intestinal metaplasia and blood stasis(OR>1,P<0.05).Conclusion:1.Hp infection may aggravate the severity of "Qi deficiency,dampness,blood stasis,Yang deficiency,phlegm and heat" in the syndrome elements of chronic atrophic gastritis;In addition,it may aggravate the damage of "spleen" in the syndrome elements of disease location.Therefore,clinical syndrome differentiation should pay attention to the disease nature and location that Hp infection may aggravate the damage,so as to prevent the development and transformation of the disease.2.Hp infection in patients with chronic atrophic gastritis Qi deficiency has no significant effect on pathological changes,may be related to Qi deficiency throughout the disease.The aggravation of blood stasis is the risk factor of atrophy with intestinal metaplasia,so the treatment of intestinal metaplasia can be combined with traditional Chinese medicine of promoting blood circulation and removing blood stasis.
