Objective:To observe the clinical effect of iliac bone graft and subtalar distraction arthrodesis for the treatment of old calcaneal fracture.Methods:From August 2015 to March 2018,we surveied 30 patients of old calcaneal fracture treated with iliac bone graft and subtalar distraction arthrodesis.There were 21 males and 9female with a average age of 36.2 years old(ranged 25-44),twelve of which presented with subtalar arthritic,the average follow up period was 12 months(ranged3--18).According to the Stephens-Sanders classification,there were 24 cases of type II and6 cases of type III.The VAS scale,talocalcaneal height,talar declination angle and American Orthopaedic Foot and Ankle Society(AOFAS)Ankle-Hindfoot scale was used for clinical evaluation.Results:None of the patients had infection,necrosis,impit failure and necrosis of the talus.The mean VAS scale decreased from 4.3 points preoperatively to 0.8 points at the final follow-up;The mean talocalcaneal height increased from 66.7 mm to 73.1 mm;the mean talar declination angle increased from 14.2° to 18.8°;the mean AOFAS Ankle-Hindfoot scale increased from 47.3 points preoperatively to 80.1 points at the final follow-up,compared to before and after treatment were statistically significant(P<0.05).Conclusion:Iliac bone grafting,subtalar distraction arthrodesis can significantly improve the clinical symptoms of the patients,improve the quality of life,for calcaneal malunion is a kind of effective treatment which worthy of promotion. |
