Objective: To observe the clinical efficacy and safety of internal thermal acupuncture for the treatment of ankylosing spondylitis in the middle and late stages,and to provide a reliable basis for clinical application.Methods: In this study,a total of 60 cases were collected and randomly divided into internal heat needle group and milli-acupuncture group,30 cases in each group.The internal thermal needle group was treated once every 2 weeks for a total of 2 to 4 treatments.Acupuncture group was treated 5 times a week,2 weeks was a course of treatment,a total of 4 courses were treated.The VAS scores,main symptoms and signs(thoracic activity,occipital distance,finger distance,Schober test),laboratory index(Erythrocyte sedimentation rate,C-reactive protein),Bath ankylosing spondylitis were recorded before and after treatment in both groups.Functional Index(BASFI),Bath Ankylosing Spondylitis Disease Activity Index(BASDAI),Classification and Quantification Quantitative Score of Traditional Chinese Medicine Symptoms,and ASAS20 Standard.SPSS24.0 comprehensive analysis was used to assess clinical efficacy..Results: 1.Baseline comparison Age,gender,disease duration,stage of disease,major symptoms and signs,VAS scores for spinal pain,laboratory indicators(ESR,CRP),BASFI,BASDAI were all compared for two groups before treatment,and it was found that they all have no statistical significance.The baselines of two groups were similar and comparable.2.Comparison of curative effect The total efficiency of treatment group and control group was 86.67% and 73.33% respectively by clinical evaluation of traditional Chinese medicine.The difference was statistically significant(P<0.05);The ASAS20 compliance rate of treatment group and control group was 76.67% and 50% respectively and the difference was statistically significant(P<0.05).3.Comparison of main signs(Thoracic activity,modified Schober test,finger distance,pillow wall distance)and spinal pained VAS score Intra-group comparison: The difference before and after treatment for two groups was statistically significant(P<0.05).Comparison between groups: The difference of modified Schober test,finger distance,pillow wall distance and spinal pain VAS score for two groups was statistically significant(P<0.05);The difference of thoracic activity for two groups was not statistically significant(P>0.05).4.Comparison of blood sedimentation and C-reactive protein Intra-group comparison: The difference of blood sedimentation and C-reactive protein before and after treatment for two groups was statistically significant(P<0.05).Comparison between groups: The difference of blood sedimentation and C-reactive protein for two groups was not statistically significant(P>0.05).5.Comparison of BASFI and BASDAI The differences of BASFI and BASDAI intra-group and between groups both were statistically significant(P<0.05).6.Security analyses 1 case of treatment group appeared flustered and released after timely counseling.2 case of control group appeared delayed needle and released after treated.All of them had no any serious consequences and it showed that both therapies had good safety.Conclusion: Internal thermal acupuncture and acupuncture for the treatment of advanced ankylosing spondylitis had good efficacy and safety.However,internal thermal acupuncture therapy had a significant effect in improving the motion of spinal joints,relieving pain symptoms,reducing the disability rate,and delaying the progression of the disease.It could be seen that internal thermal acupuncture for the treatment of patients with ankylosing spondylitis could obtain satisfactory clinical efficacy and safety,and it was worth in-depth understanding and application.
