Objective: Analysis of the clinical characteristics and prognostic factors of diabetics with non muscularis invasive bladder cancer(NMIBC).Method: This study reviewed the retrospective analysis of 245 Cases of bladder patients in Department of Urology,affiliated to Qingdao University,January 2012-2013 December.All patients were treated with transurethral resection of bladder tumor(TURBC)in our hospital.The postoperative pathological diagnosis was NMIBC and standardized treatment after operation.The patient's data were collected and the medical records were traced and followed up by telephone.We divided it into non diabetic group(162 cases)and diabetes group(83 cases).And the SPSS19.0 software was used to analyze the data and compare the clinical features of the diabetic group.We used the Logrank method to analyze the single factor and made the survival curve by Kaplan-Meier method,and applied the Cox regression analysis to the multivariate analysis.Finally,we concluded the related influencing factors of recurrence-free survival(RFS),progressionfree survival,(PFS)andcancer-specific survival,(CSS)in patients with diabetes mellitus and NMIBC.Results: There were 83 cases of diabetes in 245 patients with NMIBC.And The follow-up time was 7-71 months,the median follow-up time was 56 months.In NMIBC patients with diabetes,there were 35 cases of relapse,about 42.1%,19 in progresses,22%,and 12 for tumor death,accounting for 14.4%.Compared with patients with simple NMIBC,the clinical characteristics of women with diabetes mellitus,hypertension,recurrence and progression were significantly higher.And all of them have statistical significance(P<0.05).We used Cox regression and multivariate analysis to identify the factors that influence the shorter RFS of NMIBC patients with diabetes mellitus.The factors were higher stage,BMI > 25,and no metformin(P < 0.05).And the factors affecting the shorter PFS were female,tumor multiple,higher tumor level and staging,and non metformin(P < 0.05).And the factors affecting the shorter CSS were tumor multiple,higher tumor level and staging,and non metformin(P < 0.05).Conclusion: In patients with NMIBC,the recurrence rate and progression rate of patients with diabetes was significantly higher than those without diabetes,and had statistical significance(P < 0.05).In NMIBC patients with diabetes,higher stages and non metformin are the factors affecting the tumor RFS,PFS and CSS.And BMI > 25 was the influence factor of tumor RFS,but had no effect on tumor PFS and CSS.The femalepatients have shorter RFS and PFS;and the multiple occurrence of the tumor is a factor of the shorter CSS.
