A Comparative Study Of Three Kinds Of Rhinoplasty On Improvement Of Nasal Aesthetic Angle

Purpose:1.To compare the indications and the effects of the three surgical methods to provide the basis for the selection of the nasal plastic surgery by standard photo data collection.These three operation methods include endoscopic nasal plastic simultaneous correction of nasal septum deviation,ear cartilage and cartilage of nasal septum prosthesis combined with comprehensive rhinoplasty and cartilage joint prosthesis comprehensive rhinoplasty.2.To evaluate the application of nasal tip aesthetic measurement in rhinoplasty,find out the shortcomings of the operation method and put forward the solution.Methods:The first step:(1)We had a retrospective analysis for those patients who had wry nose deformity combined with nasal septum deviation because of congenital development or acquired trauma and visited the Affiliated Hospital of Qingdao University and had a endoscopic nasal plastic simultaneous correction of nasal septum deviation from January 2013 to November 2017.(2)We had a retrospective analysis for those patients who had flat nose,nose hypertrophy,not obvious blunt nose or prominent nose over curliness and visited the Qingdao Doctor Hospital and had a comprehensive rhinoplasty of auricular cartilage and nasal septum cartilage combined with prosthesis or combined costal cartilage prosthesis for rhinoplasty from January 2013 to November 2017.All patients were followed up for 3 months.The second step: To take the position and side photograph of the patient before and after the operation.The photos were analyzed by Adobe Photoshop CC 2018 software.And separate calculate the skew angle of the nose beam,protrusion of the tip of the nose,nasal frontal and nasolabial angle(tip rotation)of the patients before and after the surgery.We evaluated the results of these three different operation methods and the improvement of the different aesthetic angles and put forward the best indications of three kinds of operation.Result:1.For patients who had a endoscopic nasal plastic simultaneous correction of nasal septum deviation the skew angle of the nose beam was 3.8±1.3°in preoperative and 1.5±0.6°in postoperative.And the difference had significant statistical significance(p=0.00<0.05).The protrusion of the tip of the nose was 0.44±0.13°in preoperative and 0.51±0.12°in postoperative.And the difference had significant statistical significance(p=0.044 < 0.05).The nasal frontal was 135.8±6.7°in preoperative and 135.2±6.5°in postoperative.And the difference did not have statistical significance(p=0.712 > 0.05).The nasolabial angle(tip rotation)was 113.3±9.0°in preoperative and 113.2±8.1°in postoperative.And the difference did not have statistical significance(p=0.98>0.05).2.For patients who had a ear cartilage and cartilage of nasal septum prosthesis combined with comprehensive rhinoplasty,the skew angle of the nose beam was 0.5±0.5°in preoperative and 0.3±0.4°in postoperative.And the difference did not have statistical significance(p=0.063>0.05).The protrusion of the tip of the nose was 0.36±0.09°in preoperative and 0.44±0.07°in postoperative.And the difference had significant statistical significance(p=0.00 < 0.05).The nasal frontal was134.6±6.5°in preoperative and 144.2±6.9°in postoperative.And the difference had significant statistical significance(p=0.00 < 0.05).The nasolabial angle(tip rotation)was 129.2±8.5°in preoperative and 121.3±3.0°in postoperative.And the difference had significant statistical significance(p=0.00<0.05).3.For patients who had a cartilage joint prosthesis comprehensive rhinoplasty,the skew angle of the nose beam was 0.4±0.4°in preoperative and 0.39±0.10°in postoperative.And the difference did not have statistical significance(p=0.534>0.05).The protrusion of the tip of the nose was 0.27±0.09°in preoperative and 0.39±0.10°in postoperative.And the difference had significant statistical significance(p=0.02<0.05).The nasal frontal was 137.5±3.3°in preoperative and 144.6±1.8°in postoperative.And the difference had significant statistical significance(p=0.00<0.05).The nasolabial angle(tip rotation)was 131.3±7.5°in preoperative and 122.8±6.0°in postoperative.And the difference had significant statistical significance(p=0.02<0.05).Conclusion:1.Endoscopic nasal plastic simultaneous correction of nasal septum deviation can obviously improve the nasal ventilation function of the patients,meanwhile,it can effectively correct the deviation of the nasal bridge,and also has a certain effect on the raising of the nose tip,but it has no obvious effect on improving the height of the nasal bridge.2.Ear cartilage and cartilage of nasal septum prosthesis combined with comprehensive rhinoplasty has a great improvement in the shape of the nose and nose,but it has little effect on the skew of the nose.3.The postoperative effect of cartilage joint prosthesis comprehensive rhinoplasty is similar to the ear cartilage and cartilage of nasal septum prosthesis combined with comprehensive rhinoplasty.This operation improves the shape of the nose and nose,and the costal cartilage can provide more cartilage and better cartilage.This operation is more obvious than the ear cartilage and cartilage of nasal septum prosthesis combined with comprehensive rhinoplasty in the improvement of nasal tip protrusion.It is especially suitable for the serious collapse of the nose bridge and the lack of self ear cartilage and nasal septal bone or poor quality.It is a way of operation worth promoting.
