| ObjectiveNasal septoplasty is a common operation for the treatment of nasal septum deviation.After nasal septoplasty,the traditional method of bilateral nasal cavity packing can effectively stabilize the nasal cavity structure and prevent the occurrence of nasal cavity adhesion,but it brings great discomfort to the patients after operation.Some studies have shown that septal suture after septoplasty can effectively close the nasal septum mucosa space,and patients feel less uncomfortable.In this study,visual analogue scale(VAS)was used as the subjective index,and serum C-reactive protein(CRP)and interleukin-6(IL-6)were used as objective indexes to evaluate the application effect of nasal septum suture and nasal cavity packing in septoplasty,especially to compare the degree of postoperative discomfort caused by the two kinds of intraoperative treatment,so as to provide reference for clinical practice.MethodsRetrospective analysis of 40 patients treated with nasal endoscopic septoplasty between October 2019 and March 2020,suture group(20 patients)and packing group(20patients).Suture group was given nasal septum suture,and packing group was given nasal cavity packing.On the 2ndday after surgery,the scores of subjective discomfort(8dimensions of nasal pain,headache,dizziness,nasal congestion,nasal itching,tearing,runny nose,sleep difficulty)were evaluated by using the visual analogue scale(VAS).The serum CRP level was detected by immunoturbidimetric method,and the serum IL-6 level was detected by enzyme-linked immunosorbent assay.The differences of CRP and IL-6levels between groups and before and after operation were compared.At 3 months of follow-up,the occurrence of complications such as nasal septal perforation,nasal septal hematoma,nasal adhesions,and secondary epistaxis were observed in the two groups after surgery.SPSS 21.0 was used for statistical analysis,in which the count data was expressed as percentage,and the comparison was conducted by chi square test;the measurement data included VAS score of subjective discomfort and CRP,IL-6 were expressed by mean±standard deviation(x?s),the comparison between groups was conducted by independent t test,and the comparison within groups was conducted with paired t test.P<0.05 was considered to be statistically significant.Results1.On the 2ndday after surgery,the nasal pain VAS score,headache VAS score,dizziness VAS score,nasal congestion VAS score,nasal itching VAS score,tearing VAS score,runny nose VAS score,and sleep difficulty VAS score in suture group were significantly lower than those in packing group(all P<0.05).2.Before surgery,there were no significant differences in the serum CRP and IL-6levels between the two groups(both P>0.05).On the 2ndday after surgery,the serum CRP and IL-6 levels in suture group were significantly lower than those in packing group(both P<0.05).3.During the postoperative follow-up,there was 1 case of nasal adhesions and 2 cases of secondary epistaxis in packing group,with a total incidence rate of postoperative complications of 15.00%(3/20).There was 1 case of nasal septal hematoma and 1 case of nasal adhesions in suture group,with a total incidence rate of postoperative complications of 10.00%(2/20).There was no significant difference in the total incidence rate of postoperative complications between the two groups(P>0.05).ConclusionNasal septum suture and nasal cavity packing in septoplasty can achieve satisfactory results of nasal septum correction.The nasal cavity of patients recovered well after operation,and the incidence of complications was not significantly different.Serum CRP and IL-6 can be used as an objective means to compare the subjective discomfort of nasal septum suture and nasal tamponade.Compared with nasal tamponade,nasal septum suture is used in nasal septoplasty.The subjective discomfort of patients is more mild and the acceptance of patients is higher,which is worthy of clinical application. |
