Objective: To evaluate the changes of quality of life, anxiety, social support, and daily living ability of elderly patients with anxiety disorders before and after the treatment, and to analyze the influencing factors and explore effective intervention measures. Methods: A total of 120 anxiety disorders were recruited as anxiety group from our hospital during January 2015 to February 2017 and equally divided into control and observation groups, which treated with buspirone and fluoxetine for eight weeks, respectively.Eighty healthy persons were included in the same period as normal group.General information, clinical outcome and adverse reaction rate in each group were observed and compared.Evaluations of Hamilton anxiety scale(HAMA),ADL,social support revalued scale(SSRS),and short form36 questionnaire(SF-36)for all volunteers were conducted both before the treatment,2 weeks,4 weeks and 8 weeks after the treatment.Logistic regression analysis was used to identify the influence factors on quality of life in elderly patients with anxiety disorder. Results: 1.The anxiety group showed higher HAMA and ADL scores but lower SSRS and SF-36 score than the normal group(P < 0.05);there were statistical significance in sex, monthly income, complications, and marital status;there was no statistical significance in age, nationality, education, living condition, disease course, and disease expense(P > 0.05).2.Binary logistic regression analysis suggested that sex, degree of anxiety and daily living ability were the influence factors of elderly patients with anxiety disorder.3.There was no significant difference in age, sex, nationality, education, living condition, disease course, medical expenses, monthly income and complications between the control and observation groups(P > 0.05).4.The clinical efficiency of control and observation groups were 90.00% and 73.33%,respectively(P <0.05).5.The adverse reaction rates of control and observation groups were 20.00% and16.67%,respectively(P > 0.05).6.There was no significant difference in HAMA before treatment(P > 0.05);the scores of HAMA after treatment in both groups were significantly lower than those before treatment(P < 0.05);after 2,4 or 8 weeks of treatment, the HAMA scores in the observation group were significantly lower than thosein the control group(P < 0.05).7.There was no significant difference in ADL before treatment(P > 0.05);the scores of ADL after 4 or 8 weeks of treatment in both groups were significantly lower than those before treatment(P < 0.05);the HAMA scores in the observation group were lower than those in the control group after 4 or 8 weeks of treatment(P < 0.05).8.There was no significant difference in SSRS before treatment(P > 0.05);the scores of SSRS after 4 or 8 weeks of treatment in both groups were elevated compared to those before treatment(P < 0.05);the SSRS scores in the observation group were lower than those in the control group after 4 or 8 weeks of treatment(P < 0.05).9.There was no statistical significance in PF,RP,BP,GH,VT,SF,RE,MH and their aggregate score before treatment(P > 0.05);these parameters ascended significantly after treatment in both groups than those before treatment(P <0.05),and showed higher levels in the observation group than in the control group(P <0.05).10.The analysis of Spearman rank correlation demonstrated that HAMA was negatively correlated with SF-36(r =-0.623,P = 0.000),and that SSRS was positively correlated with SF-36(r = 0.579,P = 0.000).11.Binary logistic regression analysis showed that degree of anxiety, social support and clinical efficacy were the influencing factors of living quality in elderly patients with anxiety disorder. Conclusion: 1.The factors influencing the quality of life in elderly patients with anxiety disorder are anxiety level, social support and clinical efficacy.2.Clinical intervention can improve the quality of life while improving anxiety symptoms.3.Fluoxetine in the treatment of elderly patients with anxiety disorder is more effective than buspirone in improving the clinical efficacy, and the value of clinical application is higher.
