OBJECTIVE:To observe the difference of pain,saliva and taste between Burning mouth syndrome(BMS)patients and the normal population,and to study the effect of intramuscular injection of vitamin B1(VitB1)and vitamin B12(VitB12)on the treatment of BMS.METHODS:20 BMS patients who were treated in the Stomatology Hospital of Qingdao methods from 2016-09 to 2017-09 were selected as case group.Meanwhile,20 patients accompanying stomatology hospital were selected as normal control group.?Before treatment:We observed and compared the differences of Unstimulated Salivary Flow Rate(USFR),Stimulated Salivary Flow Rate(SSFR),salivary amylase,cortisol,SIgA concentration,taste level and anxiety between BMS patients and normal controls.?After treatment:In the case group,received intramuscular injection of the buttocks with VitB1100mg and VitB12 0.5mg,1 times a day for 10 days.The changes of the above indexes were observed and compared before and after treatment.Visual Analogue Scale/Score(VAS)was used to assess the degree of pain relief in patients and to be used as a clinical evaluation index.RESULTS:Before treatment,SSFR:the case group was significantly lower than the control group(P<0.05),USFR:the case group and the control group had no significant difference(P>0.05);the concentration of salivary amylase and SIgA:the case group were significantly lower than the control group(P<0.05),the concentration of cortisol in the case group(141.56 ng/ml)than the control group(116.11 ng/ml).But the difference was not statistically significant(P>0.05);cases of sensitivity of taste(sour,sweet,bitter,salty)were significantly decreased compared with the control group,but only the difference was statistically significant level of bitter sleep(P<0.05).(2)after treatment:the clinical efficacy of VitB1 combined with VitB12 intramuscular injection in the treatment of BMS:the cure rate was 10%(2/20),the effective rate was 40%(8/20),the effective rate was 20%(4/20),and the inefficiency was 30%(6/20).VitB1 VitB12 BMS,clinical treatment of the total efficiency was 70%(14/20);USFR were significantly increased(P<0.05),SSFR compared with before treatment,the difference was not statistically significant(P>0.05);the concentration of SIgA in saliva was significantly lower than that before treatment(P<0.05),the concentration of cortisol and salivary amylase than before treatment significantly change(P>0.05);the level of taste was improved in different degrees compared with those before treatment.In 2 patients,the level of acid taste was improved(10%),the level of sweet taste in 10 patients was improved(50%),the level of bitterness in 11 patients was improved(55%),and the level of salty taste in 4 patients was improved(20%).There was a significant difference between sweet and bitter taste before treatment(P<0.05),and there was no significant difference in anxiety level before treatment(P>0.05).CONCLUSION:?The abnormal decrease of SSFR,salivary amylase,taste sensitivity and abnormal increase of SIgA level in BMS patients may be a sensitive saliva indicator for the diagnosis of BMS.? VitB1 and VitB12 intramuscular injection is effective for patients with BMS,and the pain is relieved.?Changes in the concentration of SIgA level may be used as an indicator of the follow-up and prognosis of the treatment of BMS patients.?BMS patients may have anxiety tendency,but VitB1 and VitB12 intramuscular injection may not improve their anxiety level.
