Aim:To do comprehensive evaluation and analysis of etiological factors involved in patients with burning mouth syndrome.Methods:(1)We compared 75 BMS patients with 59 age-and gender-matched healthy volunteers about local causes,systemic causes and psychology factors.We collected local and systemic causes by means of questionnaire,clinical and laboratory examinations and assessed psychology condition using Zung Self-Rating Anxiety Scale and Zung Self-Rating Depression Scale.Multiple causes were analyzed with Chi-square test and Logistic regression analysis.(2)We chose 55 BMS patients and 26 age-and gender-matched healthy volunteers to test cold detection thresholds(CDT),warm cold detection thresholds(WDT),cold pain thresholds(CPT)and hot pain thresholds(HPT)at the central of the lower lip and the tip of the tongue,which can be used to assess neuropathy.In addition,20 patients whose clinical symptoms improved were retested after one month.Statistical analysis was performed using Student t test and Spearman correlation test.Results:Depression(p=0.009)and gastritis(p=0.004)had significant correlation with BMS.CDTs(p=0.040)and CPTs(p<0.001)at the tongue and CPTs(p=0.014)at the lower lip were significantly lower(less sensitivity)and HPTs(p=0.003)were significantly higher(less sensitivity)at the tongue in BMS patients compared to control participants.CPTs(p=0.010)after treatment were significantly higher(much sensitivity)at the lower lip and HPTs(p<0.001)after treatment were significantly lower(much sensitivity)at the tongue compared to the one before treatment.CDTs at the tongue(p=0.030)had correlation with depression.Conclusion:Psychology factors play an important role in the pathogenesis of BMS.Meanwhile,QST has an application value in the diagnosis and assessment of small-fiber neuropathy in BMS patients.
