Risk Factors Of PVP Postoperative Urinary Tract Infection

Objective:To analyze the related factors of postoperative urinary tract infection in patients undergoing transurethral green laser vaporization of prostate(PVP),and to further prevent the risk factors of urinary tract infection before surgery and improve the quality of life of patients.Methods:The patients were selected from January 2016 to January 2018in the Department of Urology of Shijiazhuang traditional Chinese medicine hospital,who were eligible for prostate hyperplasia,concurrent transurethral green laser vaporization of prostate(PVP)and complete postoperative follow-up.120 patients were followed up.120 patients 54-82 years of age,the average age was 65.5 years,treatment of preoperative administration of basic diseases,urinary tract infections after surgery will be according to the standard of diagnosis of urinary tract infection to diagnosis,all postoperative patients were divided into infection group and non infection group,analysis of risk factors for postoperative urinary tract infection,according to the age(>65years old),hypertension,diabetes mellitus,preoperative indwelling catheter,operation time(>60 minutes),time of indwelling catheter after surgery(>5days)and other related factors,to collate statistical information of patients,were analyzed by SPSS21.0 software,the normal test with normal distribution,the count data were compared with X~2 test and single factor analysis and multi factor Logistic regression analysis of single factor is different in single factor,Logistic in the process of analysis,based on stepwise regression SPSS21.0 In the method of fitting the model,the selection and elimination level of the variable is 0.10,that is,the factor of P value<0.10 can be included.If the p value is>0.10,the factor is eliminated.The difference of P<0.05 was statistically significant,and the influence of the above factors PVP on postoperative urinary tract infection was analyzed.Results:Univariate analysis of 120 cases of transurethral photoselective vaporization of the prostate postoperative urinary tract infection occurred in21 cases,99 cases of infection,the incidence of urinary tract infection rate is about 17.5%,older than 65 years old in 66 cases,16 cases of urinary tract infection,the infection rate was 24.2%,the difference was statistically significant P<0.05,and and 62 cases of hypertension,10 cases of urinary tract infection,the infection rate was 16.1%.There was no significant difference in P>0.05,47 cases of patients with diabetes,13 cases of urinary tract infection,the infection rate was 27.7%,the difference was statistically significant P<0.05,preoperative urinary retention for indwelling catheter in 46 cases,incidence of urinary tract infection in 14 cases and the infection rate was30.4%,statistically significant differences P<0.05,54 cases of patients,operation time more than 60min,11 cases of urinary tract infection,the infection rate was 20.4%.There was no significant difference in P>0.05,66patients with indwelling catheter after operation over 5 days,incidence of urinary tract infection in 15 cases,The infection rate was 20.4%,and there was no significant difference in P>0.05.Elderly patients,diabetes mellitus and preoperative indwelling catheterization all resulted in higher urinary tract infection after PVP,which is a risk factor for postoperative urinary tract infection(P<0.05).There was no significant difference in risk factors of urinary tract infection and postoperative urinary tract infection between patients with hypertension,operation time and postoperative indwelling catheter time(PVP)(P>0.05).The single factor analysis,single factor with single factor difference of Logistic regression analysis,Logistic analysis,according to the method of stepwise regression fitting model of SPSS21.0,the variables selected and eliminated level is 0.10,the value of P<0.10 factors can be included,if the p value is the>0.10 factor removed.The difference of P<0.05 was statistically significant,and the influence of the above factors PVP on postoperative urinary tract infection was analyzed.According to the stepwise regression method to fit the model of SPSS21.0,the age of the P value is 0.042,the odds ratio(OR)was 0.303,95%confidence interval was(0.095 0.959),diabetes P value is 0.011,the odds ratio(OR)was 3.889,95%confidence interval(1.367 to 11.070),preoperative catheterization P value of0.016 the odds ratio(OR),3.680,95%confidence interval was(1.272 10.642)the patient's age,diabetes,preoperative indwelling catheter is included in the influence factors,risk factors of urinary tract infection after PVP(P<0.05),which increases the risk of urinary tract infection after operation.Conclusions:1.The age of>65,age,diabetes and preoperative indwelling catheter-ization can increase the incidence of urinary tract infection after PVP,which is a risk factor for postoperative urinary tract infection.2.The clinical study found that the patients with benign prostatic hyperplasia have no obvious influence on postoperative urinary tract infection,including hypertension,operation time,and postoperatively indwelling ureth-ral catheter.3.The results of evidence-based medicine,should be carefully selected elderly patients,positive control of blood glucose of the patients,reduce preoperative indwelling catheter,to understand the related factors of patients with urinary tract infection after PVP,the urologist for prostate patients by transurethral vaporization resection to reduce the incidence of urinary tract infection,and provide a reliable theoretical basis for in order to improve the prognosis of patients.
