Objective By investigating the self-management behavior of type 2 diabetic patients and the family caregivers' knowledge,attitude and practicein Yan'an,we analyzed the influencing factors of the self-management behavior and KAP of family caregivers,thenexplored the effect of KAP of family caregivers on the self-management beha vior of patients.To provide a scientific basis for raising the level of care for famil y caregivers and establishing a patient self-management intervention model.Method Totally 530 pairs of type 2 diabetics and their family caregivers were selecte d from 3 hospitals in Yan'an City with convenient sampling methods.They were investigated by using Chinese Version Scale of the Diabetes Self-care Activities(S DSCA)and KAP questionnalires.540 questionnaires were distributed and 530 vali d questionnaires were returned,with an effective rate of 98.15%.The statistical dat e were analyzed by the Statistical Package for Social Science(SPSS)19.0,and the strategies included descriptive statistics,non-parametric test,Spearman correlation a nalysis and multiple stepwise regression analysis.Result(s)1.Family caregivers of type 2 diabetes in Yanan ?The median of knowledge was 18,the interquartile range was 4 and a score rate was 52.95%;the median of attitude was 29,the interquartile range was 5,a score rate was 72.88%;the median of practice was 12,the interquartile range was 3,and a score rate was 39.50%.?The univariate analysis showed that:(1)there were significant differences in knowledge of caregivers of different educational levels and family histories(P<0.01);age?care timeand family's monthly income werecorrelated with knowledge of f amily carers(P<0.01);(2)There was a significant difference in the attitude of caregi vers of different educational levels(P<0.05);care time and family monthly income were positively correlated with family caregiver's attitude(P<0.05);(3)There were significant differences in practice of caregivers of different genders,education leve ls,and occupations(P<0.01).Multivariate analysis showed that in the multivariate s tepwise regression model where the family caregiver'KAP and splits the total int o dependent variables,there were 9 variables that entered the equation,patient kn owledge source,medical nursing knowledge sourse,education level,care time,fa mily monthly income,uncooperative behavior of patients,occupation,psychological pressure,and network knowledge source,9 variables could explain 17.4% of the total variation.?Spearman's correlation analysis showed that there were positive correlation between knowledge and attitude(rs=0.382,P=0.000),attitude and practice(rs=0.315,P=0.000),and knowledge and practice(rs=0.281,P=0.000).2.Type 2 diabetic patients in Yan'an ?The median of self-management behavior was 31,the interquartile range w as 18,and the score rate was 42.05%.Among them,the drug dimension score rate was 82.21%,the special diet dimension score rate was 59.88%,the sport dimens ion score rate was 49.22%,the general diet dimension score rate was 45.24%,the foot care dimension score rate was 23.77%,and the blood glucose monitoring di mension score rate was 12.04%.?The univariate analysis showed that there were significant differences in sel f-management behavior among patients with different genders,education levels,oc cupations,and complications(P<0.05).The course of disease and the number of ho spitalization were positively correlated with the patient's self-management behavior(P<0.05).3.The family caregivers' knowledge,attitude,practice and total scores of KAP were positively correlated with the self-management behavior of patients with type 2 diabetes(P<0.01).Conclusion(s)1.Family caregivers of type 2 diabetes in Yanan ?The level of knowledge was in lower medium,the level of attitude was m oderate,and the level of practice was poor.?Univariate analysis showed that:(1)The younger the age,the longer the car e,the higher the family's monthly income,the higher the education level,and th e caregiver who has a family history,the higher the knowledge level was.(2)The higher the education level,the longer the care time,and the higher the family'sm onthly income,the higher the attitude level was.(3)The higher the education level,the higher the caregiver's practice level was.Female caregivers had higher levels of practice than male caregivers.Retired caregivers had higher levels of practice th an employed or peasant caregivers.Multivariate analysis showed that the patient kn owledge source,medical nursing knowledge sourse,education level,care time,fa mily monthly income,uncooperative behavior of patients,occupation,psychological pressure,and network knowledge source were main factors affecting the family c aregivers' KAP.?There were positive correlations among knowledge,attitude and practice.2.Type 2 diabetic patients in Yan'an ?The self-management behavior was in lower medium,and there was a gap in the self-management behavior of each dimension.The highest dimension was drug,and the lowest was blood glucose monitoring.?Univariate analysis showed:the lower theeducation level,the shorter the cou rse of disease,the fewer the number of hospitalizations,the fewer the number of complications,the lower the self-management behavior was.Male patients?the reti red patients had higher levels of self-management behavior.3.Patient's self-management behavior was poorly related to poor care for fami ly caregivers.
