Objective:This project intends to observe and compare the diagnosis of sports injuries in a marathon by the musculoskeletal ultrasound physicians and the sports medicine physicians,and to guide the recovery of athletes after exercise prescription and rehabilitation treatment respectively,and to investigate the real-time application value of musculoskeletal ultrasound in marathon events.Methods:Selected athletes attending the Chongqing International Marathon in the 2017 and 2018 due to lower extremity pain were randomly divided into Muscle Skeleton Ultrasound(MUSU)Physician Group and Sports Medicine(SM)Physician Group.The classification diagnosis was performed from bone damage,joint damage,muscle damage,tendon injury,ligament injury,skin and subcutaneous tissue damage,and no injury.The diagnosis of different types of injuries was compared between the two groups.The MUSU group was diagnosed according to the results of the musculoskeletal ultrasonography.The SM group selected exercise prescriptions(ice,draft,and fixation)according to the physician's diagnosis.Visual analogy scale(VAS)was used to record the pain before the diagnosis,after the on-site intervention,and after the game 24 hours.In this case,the athletes whose pain persisted or aggravated in the two groups were returned to the hospital for further magnetic resonance imaging(MRI)examination and compared with on-the-spot diagnostic results to assess whether the on-site diagnosis of the two groups matches.Results: A total of 399 athletes who had been treated for leg pain at the end point of the Chongqing International Marathon in 2017 and 2018 were collected and all participated in the competition.There were 195 patients in the MUSU group and 204 patients in the SM group.There was no significant difference between the two groups in terms of sex,age,and pre-diagnosis VAS scores(P>0.05).The injuries of the two groups were mainly based on the knee,and the difference was not statistically significant(P>0.05).In terms of the type of injury,the diagnosis rate of tendon injury and ligament injury in the MUSU group was higher than that in the SM group(P<0.05);Participants in the MUSU group had significantly higher pain relief than those in the SM group(P<0.05);After 24 hours of follow-up,the athletes with pain persisted or aggravated.Three patients in the MUSU group(all diagnosed as bone and joint injuries on site)and 8 in the SM group(including 6 on-site diagnosis of no abnormalities)returned to our hospital for MRI.The results of comparative analysis showed that three patients in the MUSU group were basically consistent with the musculoskeletal ultrasound diagnosis and MRI diagnosis.One of the 6 SM patients had MRI tips for bone damage,2 of them showed joint damage,and 3 showed ligament injury,which was inconsistent with the on-site diagnosis.Conclusions: 1.Marathon injury is more common with tendon injuries and ligament injuries.2.The musculoskeletal sonographer has certain advantages in the real-time diagnosis of tendons,ligaments,and soft tissues.3.The use of musculoskeletal ultrasonography helps physicians to judge injuries faster and more accurately,and guides the next step in exercise prescriptions and rehabilitation treatment more effectively;4.Musculoskeletal ultrasound is real-time,reliable,convenient,and has application value in marathon and other sports events.
