Objective:To investigate the relationship between plasma B-type brain natriuretic peptide(BNP)and SYNTAX score of coronary artery lesions in patients with left main and / or three-vessel lesions.Methods:1.From November 2015 to December 2017,180 patients with left main and three branch lesions were selected,and all patients were diagnosed by coronary angiography.The mean value of SYNTAX score was calculated by two interventional physicians using Syntax score software.According to the results of the score,60 cases with SYNTAX score ? 33 were divided into 60 cases with SYNTAX score of 23 to 32.60 cases with SYNTAX score ? 22 were divided into 60 cases.The baseline data of each group,medical history and medication history were recorded,and routine examination was completed before coronary angiography,including blood and urine.Stool routine,liver and kidney function,coagulation time,blood lipid,blood glucose,chest film,heart ultrasound,brain natriuretic peptide BNPs troponin I,C-reactive protein(CRP),etc.2.According to the SYNTAX integral and baseline data as variables,the patients' SYNTAXII score was calculated and divided according to the level of SYNTAXII score.There were 49 cases with low synectadXII score from 0 to 20 points,and 72 patients with moderate synectadXII score from 21 to 31 points.In the high score group,59 patients with SYNTAXII score ? 32 and 180 patients with coronary artery disease were treated with coronary stents implanted with PCI CABG for 12 months.The incidence of adverse cardiovascular and cerebrovascular events(new stroke,re-revascularization,angina pectoris,death)were analyzed.The data were included in SPSS19.0 software to analyze the relationship between plasma B-type brain natriuretic peptide(BNP)and the changes of adverse cardiovascular and cerebrovascular events after coronary artery disease.Results::1.There was no significant difference in the SYNTAX score between the two groups(P>0.05),and the plasma BNP in each group was significantly higher than that in the SYNTAX group(P<0.05),and the difference was significant(P<0.05)between the two groups(P<0.05).The plasma BNP in the groups with SYNTAX ? 33 was significantly higher than that in the group with SYNTAX score?22.The plasma BNP in the group ? 33 was significantly higher than that in the group with SYNTAX score 23-32(P<0.05),and was significantly higher than that in the group with SYNTAX ? 33(P<0.05),which was significantly higher than that in the group with SYNTAX score ? 33(P<0.05),and the difference between the two groups was significant(P < 0.05).There were significant differences between the two groups(P<0.05).SYNTAXII integral : sex,age,smoking,hypertension,There was significant difference in plasma BNP between the two groups(P<0.05).The plasma BNP of the two groups was significantly higher than that of the groups with SYNTAXII score(P<0.05).The plasma BNP in the groups with SYNTAXII score?32 was significantly higher than that in the groups with SYNTAXII score?32,and the plasma BNP was significantly higher than that in the groups with SYNTAXII score ? 32 or more than that with the SYNTAXII score of 21NTA12.The difference was significant(P<0.05,P<0.05),and the difference between the two groups was significant(P<0.05),and the difference between the two groups was significantly higher than that in the groups of SYNTAXII score(21/31)and SYNTAXII integral(? 32).The difference between the two groups was significant(P<0.05).2.By univariate correlation analysis,the SYNTAX scores of patients with left main and / or three-vessel lesions were positively correlated with TGV LVEDDV CRP Ccrn BNPs(P < 0.05),and negatively correlated with EF(P < 0.05).By univariate correlation analysis,the SYNTAXII score of patients with left main lesion and / or three-vessel disease was positively correlated with the age of LVEDDD,and plasma BNP was positively correlated(P < 0.05),and negatively correlated with EF(P < 0.05).There was a positive correlation between plasma BNP and BMI in patients with left trunk disease and / or three-vessel disease,and a positive correlation between plasma BNP and BMI,and a negative correlation between plasma BNP and EF(P < 0.05)and a negative correlation with EF(P < 0.05).Q-Q map was used to analyze the plasma BNP in patients with BNP,and Log conversion was used to confirm the increase of plasma BNP index in patients with left main and / or three-vessel disease.The SYNTAX score of patients would also rise with it,which was independent of the correlation between the index of plasma BNP and that of patients with left main and / or three-vessel lesions.Correlation analysis with SYNTAXII score of patients,the increase of plasma BNP index SYNTAXII score will also increase independent correlation.3.Logistic regression analysis showed that the independent risk factors for predicting the severity of coronary artery disease(BNP(Exp B 1.85 P0.000)were high blood pressure,high blood pressure,high blood pressure,low density lipoprotein 3.12,low density lipoprotein 3.12,and plasma BNP(Exp B1.85,P0.000),and triglyceride 4.02,P0.023,low density lipoprotein 3.53,P0.012,and plasma BNP(Exp B1.85,P0.000).There were statistically significant differences.The independent risk factors for predicting the complexity of coronary artery disease and the score of SYNTAX ?were estimated as follows: age: Exp BU 1.22 g / P0.001 g,hypertension: Exp BU 10.72 P 0.012,triglyceride 4.36 hp 0.015,low density lipoprotein exp B1.28 p 0.017 C reactive protein Exp B1.63P0.008,plasma BNP(Exp B1.91 g / P0.000),the risk factors of coronary artery disease are as follows:(1)the risk factors of coronary artery disease are as follows:(1)the risk factors of coronary artery disease are as follows:(1)the risk factors of coronary artery disease are as follows:(1)there is no significant difference between the two groups(P< 0.01).The correlation between SYNTAX score and plasma BNP was compared horizontally,and the odds ratio of plasma BNP level to SYNTAX ?score was higher than that of SYNTAX ?score.The results showed that SYNTAX ?score was more accurate in predicting left main coronary artery disease and / or three vessel coronary artery disease.4.The incidence of adverse cardiovascular and cerebrovascular events in 180 patients with coronary artery disease and / or three-vessel disease was 29.44%,including 10 cases of new stroke,5.5656%,8.89% of re-revascularization,12.2222 cases of angina pectoris,2.78% of death and 2.78% of SYNTAXII score.The incidence rate of adverse cardiovascular and cerebrovascular events in 21/31 and SYNTAXII ? 32 groups was significantly higher than that in 20/20 SYNTAXII integral /20/32/32 and SYNTAXII/21/31 groups.There was a significant difference between the two groups(P<0.05).The SYNTAXII score and the plasma BNP level were significantly higher in the already infected group than in the non-#en2# group.It was confirmed that the plasma BNP level and the SYNTAXII score increased.The risk of adverse cardiovascular and cerebrovascular events may increase in patients with major and / or three-vessel disease after coronary artery disease.Conclusion:1.The SYNTAX score and SYNTAX score of patients with left main ?and / or three-vessel disease increased,and the plasma BNP increased.2.There was an independent correlation between the complexity of coronary pulsation disease and plasma BNP.Plasma BNP level can reflect the severity of coronary artery disease.3.Plasma BNP is more valuable for predicting the complexity of artery disease and SYNTAXII score,Plasma BNP can be used as an independent predictor of MACCE events in patients with left main coronary artery disease and/or three vessel disease.
