| Objective: The purpose of this study was to explore the correlation between BNP and FMD and the degree of coronary artery stenosis,and whether it could provide important diagnostic value for the occurrence of coronary heart disease.Methods: We randomly recruited patients who were hospitalized in the department of cardiology,affiliated hospital of qinghai university from November 2017 to November 2018 and underwent coronary angiography and non-invasive high-frequency ultrasound FMD examination.According to the international society of cardiology guidelines for the clinical diagnosis and treatment of coronary heart disease(CHD)issued in 1979,hospitalized patients with coronary angiography images,such as coronary stenosis with the area greater than or equal to 50%,are classified as CHD group.In contrast,hospitalized patients with less than 50% the area of coronary stenosis were classified as non-CHD patients.Meanwhile,according to the number of main coronary artery lesions,namely: anterior descending branch,circumflex branch,right coronary artery,divided into three groups(one,two,three).If left main coronary artery disease would be considered as two-vessel disease to be treated.Finally,83 patients were included in the CHD group and 41 patients were included in the non-CHD group.Clinical data of all subjects were collected,including general information,basic disease biochemical examination,etc.In addition,coronary artery stenosis was scored according to the Gensini score and repeated twice.BNP leve land brachial artery FMD were detected in strict accordance with the standard operating procedures,and the correlation between BNP,FMD and Gensini score was analyzed.Results:1.The following items in the CAD group were higher than those in the non-cad group(p<0.05),including smoking rate,prevalence of diabetes,hypertension,BMI,TG,TC,UA,HCY and LDL,while HDL was lower than those in the non-CAD group(p<0.05).Gensini score and BNP level in CAD group were higher than those in non-cad group(p<0.001),while FMD value in CAD group was lower than those in non-cad group(p<0.001).2.CAD group was divided into one,two or three branch groups according to the number of coronary artery disease.The Gensini score and BNP level increased with the increase of the number of coronary artery disease(p<0.001),while the FMD value decreased with the increase of the number of coronary artery disease(p<0.001).3.In the Gensini group,the Gensini score of one,two and three branch groups were 37.33±15.17,54.12±17.20 and 64.25±14.25,respectively.Compared with the the two and three branch groups,the Gensini score of one branch group was significantly lower(p < 0.05).Compared with the two branch groups,the Gensini score was significantly increased in the three groups(p < 0.05).4.In the BNP group,the level of BNP in the one,two and three branch groups were 226.25±53.10ng/l,250.03±47.19ng/l,289.60±46.16ng/l,respectively.Compared with the three branch groups,the BNP level in the one branch group was significantly lower(p < 0.05).5.In the FMD group,the FMD values of brachial arteries in the one,two and three branch groups were 7.66±0.62,6.6±0.76 and 6.03±0.75,respectively.Compared with two or three branches,FMD value in one group increased gradually(p < 0.05).Compared with the two branch groups,the FMD value of the three groups was significantly reduced(p < 0.05).6.There was a positive correlation between serum BNP level and Gensini score in CAD group(r=0.861 p < 0.001).There was a negative correlation between brachial artery FMD value and Gensini score(r=-0.856 p < 0.001).7.When BNP level was 176ng/l,the AUC was 0.907,with a specificity of 92.7% and a sensitivity of 83.1 for the diagnosis of CHD.8.When FMD value was < 6.6%,the AUC was 0.873,with a specificity of 85.4% and a sensitivity of 85.5% for the diagnosis of CHD.Conclusion:1.Compared with the non-chd group,smoking rate,hypertension rate,diabetes rate,TC,TG,LDL,UA and HCY were higher in the CHD group,and HDL was lower.2.In patients with CHD,BNP level was positively correlated with the degree of coronary artery stenosis,and brachial artery FMD value was negatively correlated with the degree of coronary artery stenosis.3.When the BNP level was greater than 176ng/l or the brachial artery FMD value was less than 6.6%,it had a good diagnostic value for CHD. |
