Related Research Between Plasma Brain Natriuretic Peptide And The Extent Of Coronary Artery Lesions

Objective To evaluate plasma BNP levels in patients with stable anginaunstable angina or acute myocardial infarction,and to investigate the relationship between the BNP levels and the severity and extent of involved coronary vessels.Methods 81 patients with coronary heart disease patients enrolled in this study.According to clinical manifestation,electrocardiography and the levels of biochemical markers of myocardial necrosis,they were classified into 3 groups: stable angina group(SAP, n=22),unstable ang-ina group(UAP,n=28)and acute myocardial infarction group(n=33. 81 subjects who underwent coronary angiography and calculated Gensini score. 20 cases of the control group (CG) was not found any emarked coronary atery stenosis.CAS group was divided into single-vessel(n=21),double-vessel(n=31) and tripple-vessel group(n=29).The plasma example was collected form aorta by arteriongraphy catheter. Radioimmunoassay was used to assess the level of plasma BNP.The BNP levels were compared between CAD groups and control group,and analysised with the conorary lesion vessel and score.Results The level of BNP was significantly higher in AMI groups than in other three groups(P<0.05),and was marked higher in UAP than in SAPor control( P<0.05),also was higher in SAP than in control groups (P<0.05).In AMI group had a significant correlation between the level of BNP and troponin I(TnI)(r=0.632,p<0.01).The level of plasma BNP amongthe lesions of 1,2,3 vessels were 78.09±32.29pg/ml,155.04±82.59 pg/ml,381.29±118.51pg/ml.With the multiplictical comparison and The dependablity analysis between the level of BNP and Gensini Score(r=0.876, p<0.01),we found the level of BNP tend-ed to increase withthe aggravation of coronary artery stenosis. TIMI flow showed a signi-ficant negative correlation with plasma BNP levels(r=-0.575,P<0.01).For the patients with BNP>80pg/ml,most of them were with AMI or multi-vessel disease.Conclusion The more serious the extent of coronary artery lesions were,the higher the BNP level were.The degree of myocardial ischemia caused by coronary artery lesions was correlated with the BNP level. Plasma level of BNP levels can evaluate the risk of CAD.
