Analysis On The Hospitalization Expenses And Influencing Factors Of Operation Patients With Hypopharynx Malignant Tumor

Objective:With the development of the national economy,the people's living standards have been greatly improved,and the general public increasingly demand medical care.The growth of medical expenses has been enormous.American scholars developed a controlling medical expense for short inpatient case mix tools disease Diagnosis Related group(Diagnosis Related Groups,DRGs),which is a way of case mix,namely,according to the disease Diagnosis,treatment,complications or complications,age,gender,disease severity and outcome,according to divide the patients in the hospital medical record information into several Groups of Diagnosis management system.The two functions of DRGs :(1)control the medical expenses;(2)medical performance evaluation.DRGs was born in the United States and prevailed in Europe and America.Beijing introduced the Australian version of DRGs in 2004,and the national health and family planning commission produced the national version of the DRGs on the basis of the Beijing edition,which required it to be extended to all eligible provinces.Currently,the payment method of single disease is a simplified DRGs payment method,and there are many researches on the factors affecting the hospitalization cost of single disease.Multiple studies have shown in recent years,and some disease hospitalization expenses with age,sex,hospitalization days,major clinical diagnosis,surgery,the severity of disease,complications related to such factors as the complications and outcome.Although hypopharynx malignant tumor is rare and as a special disease with high malignant degree and higher technology demand of surgical treatment,there is few studies about the hypopharynx malignant tumor patients' hospitalization expenses and influencing factors.Therefore,this study based on a 3 armour hospital in liaoning province from 2013 to2016 247 cases of hypopharynx malignant tumor surgery patients medical record data were retrospectively analysis,study of the formation of hospitalization expenses,trends and influencing factors,so as to provide reference to control medical expenses growth and provide theoretical basis for relevant departments to formulate health policy.Methods : A retrospective analysis of 247 hypopharynx malignant tumor cases with SPSS19.0 software and EXCEL table.In the single factor analysis,the quantitative data should be tested by independent sample t test or single factor variance analysis if the index is subject to the normal distribution difference test,and the rank and test of the normal distribution are not obeyed.Qualitative data is checked by chi-square test.Multivariate analysis was used to select the factors influencing the cost of patients with hypopharynx malignant tumor.In this paper,dummy variables are set and analyzed.Results :In the constituent rations in cost,drug costs is the highest,up to 51.38%,followed by treatment costs accounted for an average of 18.30%.The cost of consumables is on the rise,and other changes are not significant.Stepwise multiple linear regression analysis showed that hospitalization stay,hospital discharge time(year),the proportion of drugs,primary site of tumor,type of anatomic site were the influencing factors of hospitalization expenses.The hospitalization stay is the first influencing factor.The hospitalization costs of patients with long days of hospitalization are higher than those with short days of hospitalization.The hospitalization expenses of the patients with hypopharyngeal malignant tumor were positively correlated with the discharge time(year),indicating that the hospitalization expenses of the patients showed an annual increasing trend.The total cost of surgery for hypopharyngeal malignancies is related to the range of primary sites.The total cost of patients with hypopharyngeal malignant tumor surgery with a primary location of T4 is higher than that of patients with primary sites of T1 and T2.The expenses of the patients with hypopharyngeal malignant tumor was related to the anatomic location.The analysis of dummy variables in the anatomical position showed that the total cost of patients with pharyngeal malignant tumor after pharyngeal cancer was higher than that of piriformis.Conclusion: In order to control the hospitalization expenses,we should control the operation and drug cost,shorten the hospitalization stay and improve the treatment effect,so as to reduce the economic burden of the patients.At the same time,we should fully understand the uncontrollable factors and apply it to the localization of DRGs packet device,and actively explore and innovate more sophisticated control tools that meet the local disease characteristics.
