| Objective: To investigate the basic situation and hospitalization expenses of stroke patients discharged from a Grade A Hospital in Jilin Province,analyze the factors influencing hospitalization expenses of stroke patients,provide effective suggestions to reduce the hospitalization expenses of stroke patients and adjust the cost structure reasonably,at the same time,provide the necessary data and theoretical support for the relevant departments planning and policy-making.Methods: The basic information and cost information of the patients who were discharged from the Department of encephalopathy from January 1,2019 to December 31,2019 and diagnosed as stroke came from the management system of a Grade A Hospital in Jilin Province,using the method of systematic sampling,546 effective cases were obtained,build database with Excel 2010,the basic characteristics of patients and the composition of hospitalization expenses were analyzed,Spss19.0 was used to analyze the hospitalization expenses by single factor analysis and recursive system model,get the direct and indirect factors that affect the hospitalization expenses of patients,using Amos21.0 to draw the path diagram for path analysis,and the total effect order of each influencing factor on hospitalization expenses was obtained.Results:(1)Basic information of stroke patients: Male patients accounted for 58.24%,female patients accounted for 41.76%,the average age was(65.24 ± 11.52)years,the average length of stay was(12.61 ± 7.63)days,ischemic stroke accounted for 95.05%,hemorrhagic stroke accounted for 4.95%.(2)Hospitalization expenses of stroke patients:Drug fee,inspection fee and health material fee accounted for 44.38%,20.95% and 18.17%of the hospitalization expenses respectively;the average hospitalization expenses of male patients was higher than that of female patients,they were 16249.55 yuan and 15370.03 yuan respectively;the highest per capita hospitalization expenses was 65,173.35 yuan for patients with length of stay ≥29 days;the highest average hospitalization expenses of patients with hypertension and diabetes was 17230.46 yuan;the average hospitalization expenses of hemorrhagic stroke patients were higher than that of ischemic stroke patients,which were 30463.53 yuan and 15123.72 yuan respectively;the average hospitalization expenses of patients with complications was higher than that of patients without complications which were 27118.66 yuan and 13269.76 yuan respectively;the average hospitalization expenses of surgical patients were higher than those of non-surgical patients,which were 57732.53 yuan and 10226.84 yuan respectively.(3)Factors influencing hospitalization cost of stroke patients: According to the recursive system model,payment method,history of hypertension or diabetes and operation were indirect influencing factors,while gender,stroke classification,complications and operation were direct influencing factors,surgery can not only directly affect the cost of hospitalization,but also indirectly affect the cost of hospitalization through the length of stay,the influence degree of each factor on hospitalization expenses from large to small was as follows: length of stay,operation,complications,payment method,history of hypertension or diabetes,gender and stroke classification.Conclusion: The hospitalization expenses of stroke patients are mainly composed of drug expenses,examination expenses and health material expenses;the length of stay is positively correlated with the cost of hospitalization,which has the greatest impact on the cost of hospitalization;surgery can not only prolong the length of hospital stay,but also directly affect the increase of hospital costs;complications will increase the cost of hospitalization;the proportion of medical insurance payment and reimbursement has an indirect impact on hospitalization expenses;more than half of the patients have a history of hypertension,and suffering from hypertension and diabetes at the same time will increase the cost of hospitalization;male patients are more than female patients,and the average hospitalization cost is slightly higher than female patients;the incidence of ischemic stroke is much higher than that of hemorrhagic stroke,and stroke classification has a direct impact on hospitalization expenses. |
