The Construction Of The Conceptual Framework Of Dentist's Professionalism

Objectives:Since the reform and opening up,with the improvement of national economy,Culture and living standards,there's increasing demand for oral cavity health care and more expectation for oral cavity industry.Although the government has formulated a series of policies to deal with difficulties of seeing a doctor,expensive medical treatment,intensive relationship between doctors and patients,and uneven distribution of medical resources,there are still many problems to be solved in health care and health.The oral hygiene business is an important part of the whole medical and health industry,and its development situation is equally severe.At present,there are few foreign studies on the aspect of dentist's professionalism,while the domestic is basically in a blank state.Therefore,an in-depth study of the dentist's professionalism is of realistic significance to the development of oral cavity health in contemporary China.The purposes of this research are as follows:(1)To discuss the connotations and characteristics of dentist's professionalism,and to initially complete the construction of the conceptual framework of dentist's professionalism.(2)To complete the development of dentist's professionalism scale based on the conceptual framework.Methods:This research uses Chinese Journal Full-text Database(CJFD),VIP database,Pub Med,Web of Science,CNKI,Wanfang,and other full-text databases,as well as Google Scholar,Baidu Academic,etc.to systematically search domestic and foreign literature concerning dentist's professionalism and gather relevant theories and concepts as the basis for carrying out research on this topic.According to the latest findings on dentist's professionalism home and abroad,an interview outline is formulated after consultation with experts.A total of 14 top-level dental experts in Liaoning were selected to conduct one-to-one interviews on six issues.After qualitative analysis by NVivo11 software,a conceptual framework of dentist's professionalism was initially established,and then invited the experts to carry out the second revision to complete the conceptual fra mework of dentist's professionalism.Based on the research of conceptual framework,In combination with literature and expert interviews,we determined the scale entries pool,conducted small-scale pilot surveys and collected feedback,and then finalized the questionnaire for all items.By means of stratified random sampling,questionnaires were issued to 11 dental specialist institutions in a province of China.The data entry was implemented by using Epidate 3.1,and the dentist's professionalism scale was developed according to modification index from Confirmatory factor analysis.Results:1.According to the current international research,combined with 3 principles and 10 terms professional responsibility of “Professional spirit of physicians in the new eraPhysician Declaration” put forward by ABIM,ACP and European Internal Medicine Coalition in 2002 and “The evaluation framework and intrument for the medical professionalism of Chinese physicians” established by Doctor Li Honghe from China Medical University in 2017,collected experts' opinions by one-to-one interviews with 14 top dental specialists in Liaoning province,and confirm high-frequency key words through analysis of results of expert interviews by NVivo 11,confirm five dimensions of dentists professionalism,which are “respect,empathy,patient profit priority”,“communication and cooperation”,“justice and fairness”,“sincerity and trustworth,sense of responsibility,honesty and self-desplineless,conforming to codes of ethics”,“pursuing excellence,commitment to competency improvement”.The conceptual framework of dentist's professionalism has been estabilished primarily including 5 first level demensions,30 second level dimensions after the second round of experts consultation and combining experts' suggestions for revision.2.Based on the conceptual framework,the development of dentists' professionalism scale was initially identified an item pool of 62 items in 5 dimensions.After consulting with experts and several dentists,8 duplicate items were deleted.After compiling the scale items,a pilot study was conducted.In order to complete a questionnaire for dentists' professionalism,sixteen dentists participated in the language modification of the questionnaire and set inverse questions.The questionnaire included 54 questions in 5 dimensions.Stratified sampling survey was conducted in province,750 questionnaires were distributed and 677 questionnaires were returned,among which the valid questionnaires were 534 and the effective rate was 71.20%.The analysis results showed that the KMO value was 0.949,and the chi-squared value of Bartlett's test was 4360.744,P<0.001,indicating that this data sample is adequate for factor analysis.Based on the modified index(MI)results,the scale was adjusted by using confirmatory factor analysis.Eventually,the scale was formed with 16 items.The Cronbach's alpha of the final scale was 0.8756,which indicated that the scale reliability was excellent.The confirmatory factor analysis results showed that the RMSEA,the CFI and the TLI were 0.038,0.983 and 0.979.The data suggested that the construct validity of this scale was excellent.Conclusions:This thesis constructs the conceptual framework of dentist's professionalism based on the research status home and abroad.Interview experts generally believe that the conceptual framework of dentist's professionalism has many similarities with the connotation of clinicians,but there are also significant differences.Based on the understanding of professionalism of dentists in China,this study established a conceptual framework of dentist's professionalism composed of 5 dimensions and 30 second-level dimensions.On the basis of this conceptual framework,the 16-item dentist's professionalism scale has good reliability and validity,which can be used to evaluate the professionalism of dentists.This reseach provides the theoretical basis and tools for further study in the investigation and intervention research of dentist's professionalism.
