Objective: As a common disease of neurology with high incidence,acute cerebral infarction(ACI)possesses the characteristics of sudden onset and rapid progression,whose clinical manifestations include transient or permanent cerebral dysfunction.ACI is also the most common type of stroke,with incidence from 60% to 80% of all stroke.ACI become the major causes of disability and death worldwide because of its high mortality rate,morbidity,and thus become one of the focus of global attention.Apelin is a natural endogenous ligand of the G protein-coupled receptor-angiotensin-type receptor-related protein(AT1,APJ).Apelin-13 is the most common subtype in China and has been extensively studied in recent years.The distribution and role of Apelin has been gradually understood in diseases such as hypertension,coronary heart disease,heart failure,diabetes mellitus and obesity.Apelin / APJ system is widely distributed in the central nervous system,but research on the nervous system function is still relatively rare.Some researchers have suggested that serum Apelin-13 may be a prognosis factor of acute cerebral infarction.To observe the changes of serum Apelin-13 in patients with ACI and to explore the possible relationship between Apelin-13 and ACI and its clinical significance,we investigate the relationship of Apelin-13 and ACI in our study.METHODS: Eighty-two patients with acute cerebral infarction were enrolled in the Department of Neurology,Central Hospital of Benxi City.All of them were included with the diagnostic criteria for acute cerebral infarction,which was developed by the Chinese Guideline for the Diagnosis and Treatment of Acute Ischemic Stroke,and testified by CT or MRI.(1)history of stroke;(2)complicated with heart,liver,kidney,blood system,endocrine system and other serious primary disease;(3)complicated with autoimmune or neoplasm;(4)complicated with severe infectious disease;(5)history of severe trauma or surgery in recent 3 months.Control group: 30 Han healthy objects from the Benxi City Central Hospital Medical Center without blood relationship whose serum Apelin levels were measured by ELISA on the day of physical examination,while that of the experimental group was tested on the 1st,7th and 14 th day of admission.In the case group,the serum Apelin-13 level was analyzed with NIHSS score,infarct site,infarct size and m RS.Results:(1)Level of serum Apelin-13 was 25.13 ± 1.25 ng / ml on the 1st after admission,then it increased to 29.16 ± 1.36 ng / ml on 7th and 33.72 ± 0.99ng/ ml on 14 th,which still significantly lower than that of control group(38.00±1.12 ng/ml).The linear correlation analysis showed that R values were-0.912,-0.986,-0.778 respectively,which were statistically significant.P values were 0.000,0.001,0.000 respectively.(2)The NIHSS score was 10.66 ± 3.28 on the 1st day of admission in experimental group then it decreased to 7.17 ± 2.75 on the 7th day and 5.03 ± 2.81 on the 14 th day,while the level of serum Apelin-13 showed a gradual upward trend,indicating that serum Apelin-13 levels are inversely proportional to the NIHSS score and that serum Apelin-13 levels in patients with ACI are negatively correlated with their severity.(3)When ACI patients diagnosed,the infarction area,area size for testing,through the analysis of the results shows that infarction patients with its larger area significantly lower than the level of serum Apelin-13 patients with infarction area is lesser,There was a difference in serum Apelin-13 level in each group after OCSP typing,and the serum Apelin-13 level was low in the TACI group.(4)The serum Apelin-13 level was different in the two m RS groups of patients,and the serum Apelin-13 level was high in the favorable prognosis group.Conclusion:1.Apelin-13 may be a protective factor for acute cerebral infarction;2.Serum Apelin-13 levels may act as the serum marker to predict the and prognosis of ACI.
