Objectives To explore the correlation of cumulative exposure to hypertension and carotid intima-media thickness(CIMT)in the elderly.Methods A total of 3,151 participants,who aged 60 years old or over and had completed the carotid sonography examination,were finally selected from the 1,505 subjects of the Stroke Cohort and 2,235 participants of the Elderly Cohort from Tangshan Kailuan Group.There were 266 overlaping cases excluded.According to the cumulative exposure to hypertension,all participants were divided into the following four groups:cumulative exposure to hypertension for 0,2,4 and 6 years respectively.The correlation of cumulative systolic blood pressure(cum SBP)and CIMT was analyzed with Pearson Correlation and Partial Analysis.Multivariate linear regression analysis was used to analyze the correlation between cum SBP and CIMT.And the Logistic regression models were performed to analyze the influence of cumulative exposure to hypertension on abnormal CIMT.CIMT ?1.00 mm was defined as the abnormal CIMT.Results In those 3,151 participants included,2,207 cases(70%)were male and 944 were female(30%),with an average age of 68.90±7.17 years old.CIMT values in patients with the cumulative exposure to hypertension for 0,2,4 and 6 years were 0.91mm?0.94mm?0.96 mm and 0.99 mm,respectively.And the corresponding abnormal CIMT detecion rate was 33.1%?40.1%?44.5% and 52.8%.There was a positive correlation of cum SBP and CIMT indicated by correlation analysis(r=0.14,P<0.001).Multiple stepwise linear regression analysis revealed that cum SBP positively related to CIMT after adjusting for the confounding factors(B=0.02,P<0.001).Logistic regression analysis showed that cumulative exposure to hypertension for 4 and 6 years were still risk factors of abnormal CIMT after adjusting for the confounding factors.OR(95%CI)were 1.34(1.01~1.76)?1.63(1.20~2.22)respectively in cumulative exposure to hypertension for 4 and 6 years group.Conclusions CumSBP had significant positive linear relationship with CIMT.Higher cumulative exposure to hypertensin was risk factor of abnormal CIMT.
