Background: The World Health Organization lists hearing loss as a chronic disease and requires all countries to attach great importance to early prevention and treatment of hearing loss.Hearing loss is a serious hazard and has a significant impact on individuals in all aspects, including interpersonal communication, mental health and quality of life, and has become a major hidden danger to social stability.Health management plays a crucial role in the prevention, treatment and rehabilitation of chronic diseases. In view of the serious harm and heavy burden of hearing loss on individuals and society,it is imperative to cover large province-wide,well-designed,and representative large case-control studies to clarify the influencing factors of current hearing loss in Zhejiang Province and propose targeted ones.Health management countermeasures. This study intends to understand the prevalence of hearing loss in Zhejiang Province through case-control study methods, analyze hearing loss and major environmental factors, social and psychological factors, and disease factors in different populations. The relationship between the behavioral factors and the analysis of environmental and disease, environment and behavior, behavior and disease interactions on hearing loss, provide reference for the development of health management policies for hearing loss. Methods: This study mainly used the case-control study method of 1;1 paired design.A total of 1,088 cases were collected in Hangzhou,Jiaxing,Huzhou,Zhangzhou, and Lishui in In October 2016-December 2017,according to gender in the database.Agematched selection of 1088 normal hearing was used as a control to conduct a casecontrol study. The t-test was used to compare the distribution differences between the case group and the control group;the distribution differences of the categorical variables such as self-induction area,age,self-perceived work stress, and self-perceived life stress were used in the case and control groups using the paired ?2 test.Factors related to distribution differences(P<0.05)in the univariate analysis were included in multivariate conditional logistic regression analysis.Hearing loss was used as the dependent variable, and related influence factors were used as independent variables. The relationship between environmental factors, social and psychological factors, disease factors, and behavioral factors and analysis of hearing loss.General Multifactor Dimensionality Reduction(GMDR)is used to explore possible interactions between disease-behavior,environment-behavior,and behavior-behavior.At the same time explore the cumulative effects of behavioral risk factors and disease risk factors on hearing loss. Results: Smoking, drinking, excessive headphone volume when using headphones, etc.are risk factors related to the incidence of hearing loss;good physical exercise and healthy eating habits are the protective factors of hearing loss;chronic diseases such as hypertension are all hearing Loss risk factors;external environment, noisy work environment, passive smoking environment,a larger work stress environment and life stress environment will increase the incidence of hearing loss. The GMDR analysis found that there may be interactions between the environment and behaviors that affect the occurrence of diseases.Among them, the best model of the first-order is life stress, the second-order is passive smoking and life stress, the third-order is the family's average monthly income, passive smoking and life pressure. The risk cumulative effects of related diseases on hearing loss analysis included a total of hypertension and other diseases as evaluation objects. The results showed that there was a positive correlation between the accumulation of related disease risks and the risk of hearing loss. The more types of individual-related diseases, the more susceptible to hearing loss. The higher. There was a positive correlation between the cumulative risk of related behaviors and the risk of hearing loss. The more individuals with risk behaviors, the higher the susceptibility to hearing loss. Conclusion: The health management of hearing loss needs to correct bad living behaviors such as smoking, drinking, use of headphones when the volume is too high, reduce the cumulative effects of behavioral risk factors on hearing loss, maintain regular physical exercise and healthy eating habits.Reducing the interactions between age and family average monthly income, passive smoking, and stress in life, etc.Strengthen the prevention and control of chronic diseases related to hearing loss, such as hypertension, hyperlipidemia, diabetes, and migraine, and promote self-health while reducing the cumulative effects of related diseases on hearing loss.
