Objective:To investigate the value of bronchoalveolar lavage(BAL)in the treatment of children with Mycoplasma pneumoniae pneumonia.Methods : Through retrospective study of 113 children with Mycoplasma pneumoniae pneumonia admitted to the Children's Respiratory Department of the First People's Hospital of Chenzhou from April 2016 to April 2017,they were divided into the control group of 63 cases and the lavage group of 50 cases according to whether bronchoalveolar lavage was performed or not.The control group was received conventional treatment of respiratory medicine including controlling infection,relieving airway spasm,keeping airway clear,relieving cough and phlegm,maintaining acid-base balance of water and electrolyte,rehydration and other treatment.The lavage group was treated with bronchoalveolar lavage on the basis of conventional treatment of respiratory medicine.To compare the time of clinical remission,the time of improved lung signs,the improvement of lung imaging,the clinical efficacy,the improvement of pulmonary function,the length of hospital stay,the cost of hospitalization between the two groups after treatment.Results:(1)A total of 236 children with Mycoplasma pneumoniae pneumonia were included in this study,according to inclusion and exclusion criteria,there were finally 113 patients included,including 50 in the lavage group and 63 in the control group.There was no significant difference between the two groups in the age of onset,body weight,gender,course of disease before admission,clinical symptoms before admission,laboratory test results before admission,lung imaging before treatment and lung function before treatment(P>0.05).(2)The time of disappearance of fever,cough and lung rales of the lavage group after treatment was shorter than those of the control group(P<0.05).(3)Comparison of imaging improvement rates in the lungs after 7 days of treatment between the lavage group and the control group,the lavage group was higher than the control group,the difference was statistically significant(P<0.05).(4)Comparison of clinical efficacy after 7 days of treatment between the lavage group and the control group,the lavage group was higher than the control group,and the difference was statistically significant(P<0.05).(5)We compared the pulmonary function on the 7th day after treatment,the pulmonary function was improved in the control group after treatment compared with before treatment,the difference was statistically significant(P<0.01);The pulmonary function improved after treatment in the lavage group compared with before treatment,and the difference was statistically significant(P<0.01);The improvement of lung function in the lavage group was significantly greater than that in the control group,the difference was statistically significant(P<0.01).(6)The length of hospital stay in lavage group was shorter than that in control group,the differences were statistically significant(P<0.01);The hospitalization costs in lavage group was lower than those in control group,the differences were statistically significant(P<0.01).(7)A small number of patients in the lavage group experienced minor complications such as transient cyanosis,hypoxia,postoperative low fever,and severe cough,and no serious complications occurred.Conclusions : BAL treatment of children with Mycoplasma pneumoniae pneumonia can significantly improve clinical effect,promote lung imaging recovery,improve lung function,shorten the hospital stay,reduce hospital costs.
