[Abstract] Objective:To investigate the clinical efficacy, safety and value by bronchofibroscope and bronchoalveolar lavage combining budesonide and ambroxol hydrochloride on treatment of children with Refractory Mycoplasma Pneumoniae pneumonia. Methods:104 patients with severe pneumonia were randomly divided into treatment group and control group with 52 cases in each group, patients in control group were treated with comprehensive therapy of conventional antibiotics, phlegm and nutritional support, and patients in treatment group adding bronchoalveolar lavage by bronchofibroscope combining budesonide and ambroxol hydrochloride. The positive rate of sputum examination and the clinical effect, length of hospital stay, cost of treatment, adverse reactions were compared after the treatment. Results:Under flexible bronchoscope,different degrees of damage were showed in treatment group.All patients showed obviously bronchial mucosa congestive edema. The positive rate of MP-DNA in bronchoalveolar lavage fluid in treatment group was 100%(52/52), higher than that in the control group65.4%(34/52), the difference was statistically significant (P< 0.05). Length of hospital stay in treatment group was (12.85±1.46) d, shorter than those in control group (16.19±1.77) d, The time of fever in treatment group was (4.65±1.06) d, shorter than those in control group (6.48±1.16) d, the differences were statistically significant (P< 0.05). The total effective rate in treatment group was84.6% (44/52), higher than the total effective rate in control group 46.2% (24/52), the difference was statistically significant (P< 0.05). No serious complication was found in treatment group. Conclusion:Bronchoalveolar lavage by bronchofibroscope combining budesonide and ambroxol hydrochloride on treatment of children with Refractory Mycoplasma Pneumoniae pneumonia shows satisfactory clinical efficacy, less serious adverse reaction. So flexible bronchoscopy and bronchoalveolar labage should be applied in RMPP and atelectasis cases to promote lung recruitment and improve the outcome.It is a financial, effective and safe choice for patients and it is worthy of clinical promotion and application.
