Objective: To analyze the correlation between serum CXCL16 level and NYHA cardiac function classification in patients with chronic heart failure,and compare the sensitivity and specificity of serum CXCL16 and NT-proBNP in the diagnosis of chronic heart failure.Methods: 1)In this experiment,150 heart failure patients admitted to the Department of Cardiology in the First Affiliated Hospital of South China University from March 2017 to December 2017 were selected as the observation group.At the same time,50 patients with non-heart failure were selected as the control group.According to the inclusion criteria,patients with chronic heart failure were divided into the following three groups according to the New York Heart Association(NYHA)classification: NYHA II class 50 cases,NYHA III class 50 cases,NYHA IV class 50 cases,patient history was asked in detail,and general patient data(including height,weight,heart rate,blood pressure,and abdominal circumference)were collected.Electrocardiogram,chest radiograph,and echocardiography were routinely performed.The biochemical parameters such as NT-proBNP,total cholesterol,low-density lipoprotein,high-density lipoprotein,creatinine,and uric acid were recorded within 24 hours after admission.2)The level of serum CXCL16 was detected by enzyme linked immunosorption assay(ELASA),and compare the difference of CXCL16 level between the observation group and the control group,and compare the difference of CXCL16 level between subgroups of the observation group.3)To follow-up all the subgroups in the observation group after routine treatment 1 month and 6 months after discharge and detection of CXCL16 levels,compared the differences in serum CXCL16 levels between admission、 1 month and 6 months after discharge.4)To compare the correlation between serum CXCL16 level and plasma NT-proBNP in observation group.Results: 1.Compared with the control group,the overall level of CXCL16 in the heart failure group was significantly higher than that in the control group(p<0.01).2.Compared with CXCL16 level between group in NYHA Ⅱ,NYHA Ⅲ and NYHA Ⅲ,the difference was statistically significant(p < 0.01);And NYHA level is lower than thⅡ e other two groups,NYHA Ⅲ level is lower than NYHA level,Ⅲ and the difference is statistically significant(p < 0.01).3.Compared with the the serum CXCL16 level in the group at admission,the level of it was decreased in 1 month and 6 months after discharge routine treatment of patients in the classification NYHA Ⅱ,NYHA Ⅲ and NYHA Ⅲ(p<0.05),and compared with 1 month after discharge,the serum CXCL16 level was decreased in 6 months after discharge,the difference was statistically significant(p < 0.05).4.Pearson analysis showed that the serum CXCL16 of different NYHA patients with chronic heart failure was positively correlated with NT-proBNP(r=0.401,p<0.01).5.When the serum CXCL16 level was greater than3468.35pg/ml of patients with chronic heart failure,the area under the ROC curve was 0.804,and the sensitivity prediction of heart failure was 79.1%,and the specificity was 91.3%.When the plasma NT-proBNP level was greater than 2225pg/ml,the area under the ROC curve was 0.953,and the sensitivity prediction of the heart failure was 88.6% and the specificity was 94.3%.Conclusion: 1.CXCL16 expression level increased in patients with heart failure.2.For patients with heart failure with different heart function classification,CXCL16 levels were detected after follow-up patients were discharged from hospital one month and six months.3.The specificity and sensitivity of serum CXCL16 level were lower than NT-proBNP.for predicting the severity of heart failure.
