Objective:1.To observe the differences of plasma N-terminal pro-brain natriuretic peptide (NT-proBNP) levels in patients between Uygur and Han with chronic heart failure(CHF).2.To investigate the association of NT-proBNP between the race, left ventricular erection fraction (LVEF),and left ventricular end-diastolic diameter (LVEDD).3. To explore wheher there are ethnic differences of NT-proBNP level between Uygur and Han,and the significance of NT-proBNP in the diagnosis, differential diagnosis and prognosis Uigur patients with CHF.Methods:1.Reference Framingham diagnostic criteria,from December 2009 to February 2011,we,in turn, selected 100 cases of Uygur Chinese and Han Chinese with CHF,respectively, in the Fourth Affiliated Hospital of Shihezi University School.2.According to NYHA classification, we divided them into three groups. Among them,inⅡdegree,26 patients of Uygur and 33 patients of Han;inⅢdegree,27 patients of Uygur and 36 patients of Han;inⅣdegree,47 patients patients of Uygur and 31 patients of Han.We analysized the constituent ratio of heart function of two groups of Uygur and Han withχ2 test.3.At the same time,we selected 100 patients of Uygur Chinese and Han Chinese with normal heart function,respectively, as the control group.4.After admission,plasma NT-proBNP was measured with aelectrochemiluminescence and cardiacfunction was determined with ultrasonography in all 400 patients.5.Statistic analysis was performed with SPSS 16.0.Results1. In the heart failure group and normal cardiac function group,between Uygur and Han there was no significant differenc in age, gender, body mass index (BMI) (P> 0.05,see Table 1,3). But in NYHAⅣ,the age of Han and Uygur groups were significantly different (P=0.010<0.05),and the age of Uygur patients was smaller than that of Han's.2. In the group of normal heart function, NT-proBNP levels between Han and Uygur groups were not significantly different (178.45±146.56 vs.158.59±120.13 pg/ml, P> 0.05).3. In NYHA classification classⅡ, NT-proBNP levels, between Uygur and Han group, showed no significant (P=0.243> 0.05); InⅢdegree, between Uygur and Han group, NT-proBNP levels were sig nificantly different (P=0.021<0.05);Ⅳdegree, NT-proBNP levels, between Uygur and Han group, were also significantly different (P=0.013<0.05).4. In CHF group of Uygur, NT-proBNP levels were positively correlated with the NYHA classification (r=0.850, P=0.000<0.05),and negatively correlated with LVEF (r=-0.767, P=0.000 <0.05); and positively correlated with LVEDD (r=0.642, P=0.000<0.05).5.In CHF and in the normal cardiac function group of Uygur, we set 300pg/ml as cut-off point, which reflected the sensitivity of 100%,and specificity of 84%,and positive predictive value of 86.1%(100/116), and negative predictive value 100%(84/84) for NT-proBNP for diagnosis of CHF.Conclusion:1.The difference of NT-proBNP levels was significantly related to NYHA classification grade in the groups of Han and Uygur;the higher grade of heart function, the greater the difference.Moreover, in Uygur,the age of onset of heart failure was smaller than that of Han's. 2.NT-proBNP for the diagnosis of patients of Uygur with CHF has high sensitivity, specificity, positive predictive value and negative predictive value.The plasma concentration of NT-proBNP have significant correlation with NYHA level and LVEDD and LVEF,and has high value for the diagnosis and differential diagnosis of Uygur with chronic heart failure.3.NT-proBNP levels of Uygur with heart failure were higher than those of the corresponding Han subjects,It perhaps was due to their unsuitable lifestyle, the diagnosise and treatment of CHF and its primary disease.But the sense of prevention of risk factors and the bad compliance after discharge play a greater role in the process of CHF.
