Objective:Discuss the correlation between plasma NT-proBNP (N-terminal pro-brain natriuretic peptide)concentration and cardiac functio nal class of NYHA, LVEF,LVEDD in patients with CHF (chronic he art failure), as well as the difference of NT-proBNP betwween prether apy and post-therapy in patients with CHF.Methods:1. Ninety-three patients with chronic heart failure as t he CHF group, and thirty health examination patients without organic heart disease as the control group. In the CHF group, the patients we re further divided into 3 subgroups according to the classification of the New York Heart Academy(NYHA), including 31,30,32 patients i n ClassⅠ-Ⅱ,ⅢandⅣ. Compare NT-proBNP of pretherapy with tha t of post-therapy in patients with CHF.2. Collect 3 ml blood of peripheral vein blood from each patient on the next morning after they went to the hospital, and assay the plasma NT-proBNP by enzyme immunoassay.3. Evaluate the Left ventricular ejection fraction (LVEF), left ve ntricular end diastolic diameter (LVEDD) of these patients by echocar diography.4. SPSS 17.0 was used for statistical analysis. Continuous variabl es are reported as mean±SD, comparison of means in two sets deploy independent sampler t-test, comparison of parameter between before tr eatment and after treatment deploy matched-pairs sample t-test, multi-i nterclass comparison deploy one-factor analysis of variance, The relati onships between the concentrations of NT-proBNP and the LVEF,LVE DD were evaluated by Pearson's coefficient of correlation analysis. St atistical significance was established at P<0.05.Results:1. Compare the plasma NT-proBNP concentration between CHF group and control group. It showed that the level of serum NT-proBNP was higher in patients with CHF than those in control group (P<0.05).2. Among the subgroups, plasma NT-proBNP concentration increased with the increased class of cardiac function NYHA (P<0.05).3. NT-proBNP levels were negatively correlated with LVEF (r=-0. 884, P<0.001) and positively correlated with LVEDD (r=0.819, P<0.00 1).4. Compare the plasma NT-proBNP concentration of CHF group between pretherapy and post-therapy. The plasma NT-proBNP concentration decreased obviously after treatment (P<0.05).Conclusion:1. The plasma NT-proBNP concentration in CHF group was significantly higher than that in control group. The plasma NT-proBNP concentration increased with the increased class of cardiac function NYHA. The plasma NT-proBNP concentration was negatively correlated with LVEF and positively correlated with LVEDD. It is helpful to diagnose CHF as soon as possible, and estimate the severity of CHF.2. The plasma NT-proBNP concentration was significantly decreased after treatment. Maybe monitoring the plasma NT-proBNP concentration can be used to guide CHF therapy.
