【Objective】: Using heart failure echocardiography index(HFEI), which synthesizes the multinomial parameter of echocardiography, evaluated synthetically the diastolic and systolic function of patients with chronic heart failure(CHF). The aim is to provide an objective, comprehensive and quantitative criterion for appraising CHF.【Methods】: Prospectively studying 116 patients with CHF and 24 normal controls .All the subjects were scored by HFEI, graded by NHYA and measured NT-ProBNP and Tei index. Simultaneously,the patients with NHYA II, III were received 6-minute walk test.1.Appraising the sensitivity ,specificity,positive predictive value and negative predictive value of HFEI in chronic heart failure. 2. Analyzing the correlation between HFEI and NT-ProBNP 3.Analyzing the correlation between HFEI and NYHA class. 4. Evaluated the cardiopulmonary exercise capacity of the patients with CHF by 6-minute walk test ,and respectively analyzing the predictive effect of EF,E/A,Tei index, NHYA class and HFEI to 6-minute walk test.【Results】: 1.The value of HFEI of the CHF group were evidently higher than those in control group(P<0.01),in which, the sensitivity, specificity,positive predictive value and negative predictive value of diagnosising CHF were 88.6%,91.4%,96.8%,80%, when the value of HFEI is≥3.2. The results analysed by Spearman rank correlation showed that HFEI and NT-ProBNP have remarkable positive correlation (r=0.86 ,p<0.01). The HFEI increased with the NT-ProBNP relatively 3. Positive correlation between HFEI and NYHA function class can be seen obviously also.(r=0.72 ,p<0.05).The HFEI increased with the NYHA functional class relatively. The results analysed by ANOVA showed that HFEI among NHYA classI,II,III,IV had remarkable difference(P<0.01).The HFEI of patients in NYHA class I,II,III,IV were 1.80±0.80,3.66±1.49,4.96±2.09,6.07±1.78 respectively. 4. Compared with others parameter that reflected the systolic and diastolic function of heart, HFEI and LVEF predicting 6-minute walk distance was the most effective.5. Between NT-ProBNP and the multinomial parameter of echocardiography which reflected the systolic and diastolic function of heart,Spearman rank correlation and the multiple linear regression analysis showed remarkable correlation, in which, the degree of valve regurgitation, EF and D/S effects on NT-ProBNP were the largest.【Conclusion】: It is feasible to diagnosis CHF, evaluate the degree of the CHF and predict the 6-minute walk distance with HFEI,and is worthful in generalizing in clinic.
