Part 1 Effects of two editions of guidelines on diagnosis of Late Pregnancy hypothyroidismObjective:Compare the impact of two editions of ATA's old guidelines,developed in 2011,and its new guidelines,developed in 2017,on the diagnosis and treatment of terminal hypothyroidism.Methods:A total of 104 pregnant women were selected for hospital delivery in our hospital in the late pregnancy,To divide into groups that do not need treatment and groups that need treatment according to the degree of hypothyroidism,To compare the incidence of hypothyroidism with different criteria.Selection of 104 cases of severe pre-eclampsia in late pregnancy,Cluster indicators above,To compare the prevalence of hypothyroidism in patients with severe pre-eclampsia with different criteria.Results:(1)In normal pregnant women,comparison with old guidelines,The new guidelines show a statistically significant difference in the incidence of hypothyroidism in the treatment group(P<0.05).(2)In patients with severe pre-eclampsia,comparison with old guidelines,The new guidelines show a statistically significant difference in the incidence of hypothyroidism in the treatment group(P<0.05).Conclusions:New guidelines reduce the incidence of late pregnancy hypothyroidism;New guidelines reduce the incidence of late pregnancy hypothyroidism,Avoiding the clinical overtreatment of late pregnancy,Avoiding over-treatment of late pregnancy hypothyroidism,Reduced unnecessary psychological burden and economic pressure on pregnant women with hypothyroidism.Part 2 Effects of hypothyroidism on plasma protein and blood lipid in patients with severe pre-eclampsiaObjective:To investigate the effect of hypothyroidism on plasma protein and blood lipid in patients with severe pre-eclampsia.Methods:A total of 104 patients with advanced severe pre-eclampsia who were hospitalized in our hospital,According to the criteria of the new guidelines,patients with hypothyroidism were divided into groups that do not need treatment and groups that need treatment,Through the determination of plasma protein and blood lipids in patients,Comparison of plasma protein and blood lipid in two groups.Results:(1)In patients with severe pre-eclampsia,blood pressure increased in the treatment group,the difference was statistically significant(P < 0.05).(2)In patients with severe pre-eclampsia,There was no correlation between FT4 and blood pressure(P>0.05),TSH was positive correlation with blood pressure(P<0.05);(3)In patients with severe pre-eclampsia,There was a significant decrease in TB and ALB in patients with treatment group;TC? TG?LDLC and Apo B increased significantly in patients with treatment group(P<0.05);(4)In patients with severe pre-eclampsia,The sensitivity of blood lipid index to hypothyroidism was TC > LDLC > DG > HDLC;The sensitivity of plasma lipoprotein index to hypothyroidism was Apo B > Lpa > Apo A1;The sensitivity of changes in other protein indexes of plasma was ALB > TB > GLB.Conclusions:(1)TSH was correlated with blood pressure.FT4 not related to blood pressure;(2)In patients with severe pre-eclampsia,Combined hypothyroidism may increase blood pressure,Increase the blood lipids and plasma lipoprotein,reduce other plasma proteins,To aggravate the patient's condition;(3)In patients with severe pre-eclampsia,TC has the greatest influence on blood lipid index due to hypothyroidism;ApoB has the greatest influence on plasma lipoprotein index due to hypothyroidism;ALB has the greatest influence on other protein indexes in plasma due to hypothyroidism.
