Objective: We retrospectively analyzed the clinical features,laboratory tests and prognosis of 285 adults with EBV infection,and raised awareness of EBV infection,providing evidence for the early diagnosis and treatment of the disease.Materials and methods: 1.Subjects:A total of 306 adult patients with EBV infection diagnosed at the First Hospital of Jilin University from January 2013 to December 2017 were collected.A total of 285 adult patients with EBV infection were included according to inclusion and exclusion criteria.According to the different clinical manifestations,they were divided into three groups: simple EB virus infection and infectious mononucleosis group,hemophagocytic syndrome group,malignant tumor group(including lymphoma,leukemia,myelodysplastic syndrome).2.Research content: We summarize the clinical data,disease composition,laboratory test results,treatment,and prognosis of adult EB virus infection,and analyze the clinical features and laboratory features of the EB virus infection and related diseases.3.Statistical analysis: Statistical analysis was performed using IBMSPSS statistics 21 software.P<0.05 was defined as statistically significant.Results: 1.Epidemiological characteristics:.There were 138 males and 147 females with EBV infection in adults.The male-to-female ratio was approximately 1:1.06.There was no significant difference between males and females,and it was more common in young patients,accounting for 58.9%.Adult EB virus infection can occur throughout the year without significant seasonal correlation.2.Disease composition:(1)In 285 adults with EBV infection,EBV infection was the most common,accounting for 60%,infectious mononucleosis accounted for 23.9%,lymphoma,hemophagocytic syndrome,leukemia,myelodysplastic syndrome,chronic active EBV infections accounted for 16.1%.(2)Diseases of various age groups: In the young group,infectious mononucleosis and Epstein-Barr virus infection are common.In the middle-aged group,infection with Epstein-Barr virus was the most common,accounting for 74%,malignant tumors accounted for 21%,and infectious mononucleosis accounted for 5%.In the elderly group,EBV infection accounted for 69%,and malignant tumors accounted for 31%.3.Clinical symptoms:The clinical symptoms of EBV infection in adults were fever(78.2%),lymph node enlargement(47.0%)and spleen enlargement(41.1%).4.the main complications and system involvement:Adult EBV infection is commonly affected by digestion,blood,and respiratory system,accounting for 60.4%,47.3%,and 33.3%,respectively.5.Laboratory tests:Adult EB virus infection patients accounted for 54% increase in ALT,most patients with mild liver injury,ALT> 200U/L only accounted for 21.8%,total bilirubin increased only 8.8%.6.Comparison of EBV infection with various related diseases(1)Clinical symptoms:Hemophagocytic syndrome group,EBV infection and infectious mononucleosis group were more prone to fever compared with malignant tumor disease group,and the number of fever days in hemophagocytic syndrome and malignancy disease group was significantly higher than that of EB virus infection and infectivity.The mononucleosis group.Compared with EB virus infection and infectious mononucleosis group,hematopoietic syndrome group is more likely to have large liver and splenomegaly(2)Laboratory examination:The ALT,AST,total bilirubin,direct bili rubin,lactate dehydrogenase,and ferritin in the hemophagocytic syndro me group were significantly higher than those in the other two grou ps.Fibrinogen was significantly lower than the other two groups.T he EBV DNA of hemophagocytic syndrome and malignancy was sig nificantly higher than that of EB virus infection and infectious mon onucleosis.7.Treatment and follow-up The 285 adult patients with EB virus infection were given anti viral,hemophagocytic syndrome given hormones,regular chemothera py and symptomatic treatment of cancer patients,238 cases were cu red and improved,76 cases were untreated;about 100 patients were followed up for 1-2 years.Eighteen patients died.Conclusion: 1.adult EBV infection can be the disease throughout the year,was scattered in distribution,occur in young people,with leaflets,simple EB virus infection is the most common,middle and old people infected with EB virus more likely to merge hemophagocytic syndrome,lymphoma,leukemia Other diseases.2.The clinical manifestations of adult EBV infection are fever,lymphadenopathy and splenomegaly.Epstein-Barr virus infection often involves the digestive system,respiratory system,blood system involvement,liver damage is mostly mild,often self-limited,rarely severe liver failure.3.Patients with long fever days who are infected with EBV should pay attention to monitoring changes in blood routine,EBV DNA,ferritin and lactate dehydrogenase,and pay attention to screening for malignant diseases such as hemophagocytic syndrome and lymphoma,and try to make early diagnosis.Improve the prognosis.4.The clinical manifestations of adult EB virus infection are diverse,most patients have a good prognosis,and a few patients progress to the death of malignant disease.
