Objective:To summarize the diagnostic experience of carotid body tumor,and to analyze the risk factors of carotid body tumor size and texture on the operation and the operation of carotid body tumor resection and cerebral ischemic stroke.Methods:The clinical data of 103 patients(3 patients with bilateral)carotid body tumor were retrospectively reviewed from April 2004 to July 2017.Postoperative pathological examination of 106 tumors was confirmed as carotid body tumor.103 cases of patients with preoperative preoperative diagnosis will do relevant auxiliary examination,including 84 cases of preoperative examination line color ultrasound +CT/CTA,3 cases of preoperative MRI of color ultrasound +,16 cases of patients with preoperative CT/CTA.All 106 carotid body tumors underwent surgical resection,91 underwent simple tumor resection,6 rows were resected and the external carotid artery ligation was performed,and 9 rows were resected and the internal carotid artery reconstruction was performed.To investigate the effects of carotid artery injury,the maximal diameter of 91 CBT simple tumor resection was analyzed with intraoperative blood loss and operation time.The CT values of 22 CBT resections were measured by CT data,and the CT values were correlated with intraoperative blood loss and operative time.In this group,9 patients had ischemic stroke after excision of carotid body tumor,and single factor analysis was performed on the related factors in the operation,and the risk factors of IS were screened after the operation.Adopting SPSS22.0 statistical software analyzed data.Results:18 cases of 103 patients,male cases,female 85 cases,male to female ratio is about 1:4.7,the lesions located in the left and right side incidence ratio is about 1:1,including bilateral disease(3 cases),accounted for 2.83%of all cases.In the preoperative diagnosis of 106 tumors,103 tumors were diagnosed as carotid body tumors,and 3 were diagnosed with cervical schwannoma,and the diagnostic accuracy was 97.2%.According to the data analysis,it was concluded that the maximum diameter of the tumor was linearly positive correlation with intraoperative blood loss and operation time(correlation coefficient:0.256,p=0.014<0.05).Correlation coefficient:0.226,p=0.031<0.05);The CT value of carotid body tumor was not statistically significant(p=0.459>0.05,p=0.828>0.05)in the 22 CBT simple tumor resection.The risk factors of ischemic stroke(IS)after operation of carotid body tumor were:intraoperative hemorrhage(p=0.014<0.05).Operation time(p=0.008<0.05);Internal carotid artery injury(p=0.005<0.05).Conclusion:By this group of 103 patients(106.CBT surgery)retrospective study,imaging examination is better for carotid body tumor diagnosis methods,use the method of color ultrasound + CTA can provide more at the time of diagnosis for the surgical treatment of anatomical information.For the surgical treatment of carotid body tumor,the measurement of tumor size is of certain value in predicting intraoperative blood loss,operation time and difficulty.As for the effect of soft and hard texture of CBT tumor on intraoperative blood loss and operative time,there was no correlation by measuring CT value of tumor.During carotid body tumor resection,intraoperative blood loss,duration of surgery and intraoperative internal carotid artery injury are risk factors for cerebral ischemic stroke.The prevention and treatment of intraoperative risk factors for these operations complicated with IS should be carried out throughout the surgical treatment of carotid body tumor.
