Retrospective Analysis Of The Diagnosis And Treatment Of22Patients With Carotid Body Tumor

Objective: The experience of diagnosis, surgical treatment and complication weresummerized in patients with paragangliomas.Methods: A retrospective review was performed on22cases o f carotid body tumorsdiagnosed and treated between January2009and2013.Results: All carotid body tumors were confirmed preoperatively by digital subtractionangiography,color ultrasound and CTA or MRA.6cases were bilateral carotid bodytumor,of which1case had already accept unilateral resection operation,2cases underwentstaging operation,3cases had accept unilateral resection operation.According to theanatomic relations between tumor body and carotid artery,we primarily resected24tumorsof which14cases underwent simple removal,5cases co-resection of tumor body andexternal carotid artery,1case internal carotid artery repair after co-resection of tumor bodyand external carotid artery,2cases vascular reconstruction of carotid artery afterco-resection of tumor body,internal and external carotid artery,and1case ligation ofinternal carotid artery after co-resection of tumor body.After operation,24cases were typedaccord to Shamblin classification,in which12cases typeⅠ,6cases type Ⅱ,6casestypeⅢ.After operation,4cases repectively developed hoarseness,bucking while drinkingand loll left,and the symptoms disappeared via active treatment.1case developed aphasiaand muscle weakness,and recovered completely.1case developed hemiplegic paralysis,butthe muscle force recovered to gradeⅡ with active treatment.During a follow-up period of4months to2years,2lost contact,and no death.Conclusion: Ultrasound could be used in screening,and correctly diagnosis could bemade through DSA,CTA or MRA.The patients with carotid body tumor should be operated as early as possible with suitable way selected according to the relation between the tumorbody and carotid artery.The protect of the brain should attach great importance inperioperative period.The key to provent the serious complication is to protect the cerebralnerve during operation and shorten the cerebral ischemia time.
