Objective To investigate the current status of psychological distress among cervical cancer inpatients and analyzes its influencing factors.To explore the intervention of Mindfulness-Based Stress Reduction on cervical cancer patient's psychological distress and quality of life.Method(1)The Distress Management Screening Measure(DMSM),Patient Health Questionnaire-9(PHQ-9),Generalized Anxiety Disorder-7(GAD-7)and a demographic date sheet were competed by 401 cervical cancer patients from five oncology wards in a tertiary cancel hospital of Hunan.To investigate the current status of psychological distress and analyzes its influencing factors.(2)106 cervical cancer patients with chemotherapy were assigned into the intervention group and the control group with 53 cases in each group.Both groups received psychological routine case,and the intervention group also received 8 weeks of MBSR.The Distress Management Screening Measure(DMSM)and the Functional Assessment of Cancer Therapy-Cervix(FACT-Cx)were used to assess patient's psychological distress and quality of life in two groups before the intervention and after the intervention.The Statistical Package for the Social Sciences 18.0 was used for statistical analysis.All tests were two-sided and significance level was set at 0.05.Statistical methods included descriptive statistics,t-test,chi-square,Man-Whitney U test,Fisher's exact test,Spearman correlation analysis and multiple stepwise regression analysis.Results Status survey section:(1)A total of 368 valid questionnaires were received in prevalence survey.Cervical cancer patients' DT score was(4.25±2.09),and 248 inpatients scored?4,and a significant psychological distress detection rate(DT?4)is 67.4%.(2)Spearman correlation analysis level of psychological distress in patients with cervical cancer associated with anxiety,depression,level analysis,show that cervical cancer patients with psychological distress level and the level of anxiety,depression,there were significant positive correlation(P < 0.001).(3)Multiple stepwise regression analysis showed that the main sources of cost,worry,limited physical activity,problems with partners,pain,family monthly income,age,disease staging,time and energy to do housework were the factors affecting the psychological pain of patients with cervical cancer.Intervention study:(1)Finally,46 cases were included in the final intervention group and 43 cases were included in the control group.There was no statistically significant difference between the two groups in general information,psychological pain score,total score of life quality and the scores of all dimensions(P > 0.05).(2)The psychological pain scores of the patient in two groups were compared by independent sample t test,which showed that the scores of psychological pain in the intervention group were lower than those in the control group(P < 0.05).(3)Paired t test was used to compare the longitudinal changes of psychological pain scores between the intervention group and the control group before and after intervention.The results of paired t test showed that the score of psychological pain in the intervention group was significantly lower than that before the intervention(P < 0.05),and there was no significant difference between the intervention group and the intervention group before and after intervention(P > 0.05).(4)The difference between before intervention and after intervention was compared with the independent sample t test.The results showed that the difference of psychological pain in the intervention group was(-1.78±1.36),and the score of the control group was(-0.30±0.99),and the difference was statistically significant(P < 0.05).(5)The quality of life and the dimensions of the two groups of patients were compared by independent t test.The results showed that the scores of the total quality of life and their physiological,social / family status and emotional status were significantly higher than those in the control group(P < 0.05)after the intervention.(6)The paired t test was used to compare the longitudinal changes of the quality of life before and after the intervention of the patients.The results showed that the scores of the quality of life,the physiological status,the social / family status,the emotional state and the functional status of the intervention group were significantly higher than those before the intervention(P < 0.05).(7)The independent sample t test was used to compare the difference between the intervention group patients and the control group patients of the quality of life.The results showed that the difference between the total score of life quality and the physical and emotional state of the intervention group was significantly greater than that of the control group(P < 0.05).Conclusion The psychological distress level of cervical cancer inpatients was higher than mean level of cancer patients,so we should take effective measures to reduce the psychological distress level of inpatients with cervical cancer.MBSR can effectively relieve psychological distress and improve quality of life for cervical cancer patients undergoing chemotherapy.
