Objective: Cancer can be a distressing experience for cancer patients and carers, impacting on psychological, social, physical and spiritual functioning. With the improvement of medical technology, the five-year survival rate of cancer has been significantly improved, and patients’ mental health problem have been received attention. Research has shown that people with cancer show high levels of psychological distress and its across the illness trajectory and affected areas such as therapy adherence, satisfaction with care, length of hospitalisation, quality of life and even possibly survival time. The purposes of this study were to investigate the psychological distress of the people who diagnosed with cancer, understand present status and the main problems, to explore the related factors of psychological distress and the impact of different informed conditions on psychological distress, to provide reference for clinical treatment.Methods: The investigation was consisted of 316 cancer patients who were treated in the department of Medical Oncology of Hebei province(Forth Affiliated Hospital of Hebei Medical University, Second Affiliated Hospital of Hebei Medical University, Affiliated Hospital of Hebei University, Baoding Center Hospital and 258 Hospital of Handan) from December 2013 to October 2014. Several screening scales were used to investigate which include psychological distress thermometer(DT), problem list(PL) and self-designed questionnaire including patient demographic data and disease information. All datas were entered by the Excel to establish a database and analyzed by SPSS13.0software. Square test or factor variance analysis methods was used to detect differences of general information between psychological distress group and different informed condition. Spearman correlation analysis was used to analyze the general information and psychological distress. Use regression analysis to analyze risk factors of psychological distress and informed condition. P values<0.05 were considered statistically significant.Result:1 In 336 cases of data, the incidence rate of significant psychological distress is 35.44%, and DT score were(2.88±1.77) ranged from 0~9.2 The pain score varied with age, residence, diagnosis, the existence of metastasis, PS score, current treatment phase and informed situation(P< 0.05 for all);Logistic regression analysis suggested that tumor diagnosis, treatment and informed condition were influencing factors of psychological distress;The DT score was negatively correlated with age, residence and informed condition, positively correlated with PS score and the existence of metastasis.3 Emotional and physical problems have significant effect on psychological distress. Logistic regression analysis suggested that depression, fear, worry, appearance and pain are risk factors related to distress.4 The percentage of patient who were fully informed was 51.89%, partially informed was 28.79% and uninformed was 35.48%. The DT score was significantly different among informed conditions(P= 0.008) with highest in uninformed(3.70±2.319) and lowest in partially informed(2.62±2.059). The prevalence of significant distress in different conditions(uninformed 47.54% vs partially informed 26.27% vs fully informed 35.59%, P= 0.017) have significant difference, and P value= 0.017 was considered statistically significant.Ordinal regression analysis suggested that DT score is a risk factor for informed condition(P= 0.002).Conclusion:1 The incidence rate of significant distress is relatively high in cancer patients in this region, the incidence rate of significant psychological distress is 35.44%, which should be paid more attention in the clinical work.2 The risk factors may consist of living in rural areas, breast cancer and uninformed. The main problems of psychological distress in cancer patients were emotional and physical problem.3 The DT score was negatively correlated with informed condition and was significantly different among informed conditions.Uninformed was a risk factor of psychological distress, which remind us it is necessary to screen the psychological distress in cancer patients, and we should make individualized intervention for patient with different conditions.
