Functional dyspepsia(FD)is a kind of chronic dyspepsia characterized by one or more of symptoms, such as postprandial fullness, early satiety, epigastric pain, and epigastric burning sensation. Most scholars believe that the main pathogenesis of FD is gastrointestinal motility disorders and visceral hypersensitivity.Recent studies have revealed the role of psycho-psychological factors in the pathogenesis of FD.In recent years, the role of transient receptor potential vanilloid 1(TRPV1)in anxiety, depression and other mental diseases has been discovered. In this experiment, the serum and gastric antrum mucosal TRPV1 levels were measured in FD patients and healthy volunteers. To assess their anxiety and depression mood,Nepean dyspepsia index(NDI).To analyze the relationship between TRPV1 and psycho-psychological factors of FD patients, and to explore the relationship between TRPV1 and dyspepsia symptoms and quality of life. The aim is to provide new clues for the pathogenesis, diagnosis and treatment of FD.Objective:To analyze the relationship between TRPV1 expression level and psycho-psychological factors of FD patients, and to explore the relationship between TRPV1 and dyspepsia symptoms and quality of life. Methods:To evaluate the anxiety and depression mood of FD patients and healthy checkers.Divided them into three groups: FD with anxiety and depression mood group(group A),FD without anxiety and depression mood group(group B),and healthy control group(group N).Evaluating NDI of all the objects.Serum TRPV1 concentration and gastric mucosa TRPV1 expression levels was detected. To analyze statistical data.Results1 For the SAS and SDS scores, group A,group B and group N respectively were(SAS: 51.68±7.06 vs 39.11±6.81 vs 29.96±6.45;SDS: 60.09±8.14 vs41.72±6.33 vs 29.68±5.59).Group A had the highest score and Group N had the lowest score. The differences among the three groups were statistically significant(P <0.01);2 For the level of serum TRPV1,group N,group B and group A respectively were(0.469±0.212 vs0.391±0.143 vs0.254±0.174),group N was larger than group B,group B was larger than group A.The differences among the three groups were statistically significant(P <0.01).The results of comparison between groups showed that there was significant difference in serum TRPV1 concentration between group A and group B,group N(P<0.05,P <0.01).There was no significant difference in serum TRPV1 concentration between group B and group N(P <0.173);3 For the expression of TRPV1 with gastric antrum mucosa, group A,group B and group N respectively were(4 vs 3 vs 1),group A was larger than group B,group B was larger than group N.The differences among the three groups were statistically significant(P < 0.05);4 For the NDSI and NDLQI,group A,group B and group N respectively were(NDSI: 56 vs 33 vs 4;NDLQI: 38.0 vs 75.0 vs 98.4),group A was larger than group B,group B was larger than group N.The differences among the three groups were statistically significant(P<0.01;P<0.01);5 Serum TRPV1 concentration was negatively correlated with SAS score and SDS score(?=-0.397,P<0.01;?=-0.401,P<0.01);The expression of TRPV1 in antrum mucosa was positively correlated with SAS score and SDS score(?=0.693,P<0.01;?=0.785,P<0.01);6 SAS score and SDS score were positively correlated with NDSI(?=0.749,P < 0.01;?=0.827,P < 0.01),and negatively correlated with NDLQI(?=-0.790,P<0.01;?=-0.847,P<0.01).Conclusions:1 FD patients have significant anxiety and depression mood compared with healthy people;2 FD patients with TRPV1 in antral gastric mucosa overexpression.FD patients with anxiety and depression mood with TRPV1 in antrum gastric mucosa expression higher than FD without anxiety and depression mood;3 The serum levels of TRPV1 in patients with anxiety and depression mood are lower than people without anxiety and depression mood;4 The anxiety and depression scores were positively correlated with the severity of symptoms and negatively correlated with the quality of life in FD patients;5 TRPV1 can indirectly reflect the condition of FD patients and is expected to become a marker of FD diagnosis and treatment and condition evaluation.
