Objective:To explore normal subjects and patients with the reconstruction of posterior cruciate ligament injury in the knee flexion angle of gait,tibial bone rotation angles inside and outside,inside and outside the mutual relation between flip angle varies with time.Methods: The experimental group: 30 cases from October 2015 to October 2017 in Affiliated Zhongshan Hospital Dalian University the reconstruction of posterior cruciate ligament injury patients(18 male and 12 female);age 25-45 years,the average age 32.5years;height 153-178 cm,the average 164.8cm;the weight 47-78 kg,the average weight67kg;the time of injury;4 weeks of-1 years,the average of 4.3 months.In the operation of arthroscopic posterior cruciate ligament was confirmed complete fracture,with no lateral collateral ligament injury,anterior cruciate ligament injury,internal and external meniscus with oblique horizontal tear,and before damaged the knee function was normal;the other side of the knee joint after imaging examination and experienced department of orthopedics doctor was no injury and no previous history of trauma or surgery.The control group:from the imaging examination and experienced department of orthopedics doctor find no knee injury healthy people,and no previous history of trauma or surgery history(male 18,female 12),age 26-40 years,the average of 30.7years old,height 152-180 cm,the average of 167.6cm,weight 51-77 kg,the average of62 kg.The gait analysis of the experimental group and the control group was carried out by using the Oriental New DVMC-8820 three dimensional motion capture system.Two groups of objects were collected flexion and extension angle,internal and external rotation angle of tibia,internal and external rotation angle data from walking for 15 seconds at a speed of 1.8km / h.Experimental group were collected after operation 3months,6 months,9 months,12 months follow-up analysis.Find two groups statistical significance by SPSS18.0 system of flexion and extension angle,internal and external rotation angle of tibia,Internal and external rotation angle data.Results:PCL double bundle reconstruction after 3 months the flexion and extension angle range is-3.7°-48.5°,internal and external rotation angle-7.3°-14.4°,internal and external rotation angle of tibia is-7.3°-15.1°;PCL reconstruction after 6 months the flexion and extension angle range is-3.5°-59.4°,internal and external rotation angle is-5.9°-12.8°,internal and external rotation angle of tibia is-6.5°-12.6°;PCL reconstruction after 9 months the flexion and extension anglerange is-3.2°-64.2°,internal and external rotation angle-4.9°-9.7°,internal and external rotation angle of tibia is-5.0°-10°;after 12 months the reconstruction the flexion and extension angle range is-3.1°-65°,internal and external rotation angle-5°-9.5°,internal and external rotation angle of tibia is-4.9°-9.3°;healthy knee the flexion and extension angle range is-2.9°-68°,internal and external rotation angle-4.9°-9.6°,internal and external rotation angle of tibia is-4.7°-9.3°.There was a significant difference in the rotation parameters between 3 months,6 months after operation and health(P < 0.05).There was no significant difference in the rotation parameters between 9 months,12 months after operation and health(P > 0.05).In the walking gait cycle swing phase,the tibial rotation,knee varus angle and flexion angle will reach the maximum at the same time.In the standing phase,the tibial internal rotation,knee valgus angle and knee extension angle will reach the maximum at the same time.Conclusion: Three dimensional gait analysis can be used to analyze the changes of human gait and provide a method for understanding the kinematics of knee joint after posterior cruciate ligament injury.Within 6 months reconstruction of posterior cruciate ligament,there was a significant difference in the rotation parameters for normal people.However,gait was almost the same after 9 months operation.The three-dimensional gait analysis of knee joint angle is reflected in intuitive figures,which is more objective,true,accurate and reliable,we can directly determine whether the knee joint is stable or not by using the data obtained,which can objectively evaluate the operative effect and adjust the rehabilitation training in time according to the angle changes.
