Objectives:To conduct a questionnaire survey on shoulder pain patients attending orthopedic clinics at Dalian Municipal Central Hospital Affillated of Dalian Medical University to assess the mental health status of these patients,to understand their clinical characteristics,and to explore the factors that affect the mental state of shoulder pain patients,thereby improving the efficacy and promoting the patient's clinical rehabilitation.It provides a new theoretical basis for somatic and psychological therapy in patients with shoulder pain.Methods:Patients with shoulder pain who were treated at the orthopaedic clinic of Dalian Municipal Central Hospital Affillated of Dalian Medical University from October 2016 to December 2017 were collected.A total of 413 patients with complete data were included in the inclusion and exclusion criteria.A questionnaire survey was conducted among all patients who met the criteria.The questionnaire included the Self-rating Depression Scale(SDS),the Self-rating Anxiety Scale(SAS),Visual Analogue Scale(VAS)score,and general socio-demographic data.Under the guidance of investigators,the patient himself or his family assisted in completing all the contents of the questionnaire.The recovered data was entered into an Excel database and SPSS22.0 statistical software was used for statistical analysis.Using Chi-square test to compare the rates,using Logistic regression model analysis the relationship between anxiety or depression and the patient's gender,age,marital status,education level,pain level and other factors.Results:A total of 413 eligible questionnaires were recovered in this study,including176 males(42.62%)and 237 females(57.38%).The age ranged from 39 to 78 years old,and the average age was 55.43±8.04 years.The standard score of SAS was 46.89±10.85 and the SDS standard score was 49.40±11.91.SAS ? 50 and SDS ? 53 are respectively as the demarcation criteria,133 cases(32.20%)with anxious tendency were detected in this study,among which 75 cases(18.16%)were mildly anxious and 40 cases(9.68%)were moderately anxious.There were 18 cases(4.36%)with severe anxiety,155 cases(37.53%)with depressive tendency,67 cases(16.22%)with mild depression,62 cases(15.01%)with moderate depression,and 26 cases(6.30%)with severe depression.There were 215 cases(52.06%)with anxiety or depression,including only 60 cases with anxiety(14.53%),only 82 cases with depression(19.85%),73 cases with anxiety and depression(17.68%).The VAS score was 5.58±1.44 points.Of the 413 patients,311 cases were moderate pain(75.30%)and severe pain was 102 cases(24.70%).Univariate analysis suggested that age,family economic situation and pain degree may affect the occurrence of anxiety(p<0.05),and the factors that affect depression may be marital status,education level,pain level,and family economic status(p<0.05).Multivariate logistic regression analysis suggests that age is a factor that affects the occurrence of anxiety.The risk of anxiety in married patients is higher,and the average monthly income per household is more likely to occur anxiety.The self-paid patients are more likely to have anxiety than medical insurance(including public expenses);the VAS score and the education level of the patient are factors that affect the patient's depression.Conclusions:Patients with shoulder pain who were seen at orthopedic clinics in Dalian Municipal Central Hospital Affillated of Dalian Medical University had significant anxiety and/or depression.SAS and SDS scores were significantly higher than those in the normal population.Surveys showed that female patients had slightly higher rates of anxiety and depression than male patients.The occurrence of anxiety in patients with shoulder joint pain may be related to factors such as age,marital status,family economic situation,and payment methods of medical expenses;the factors that influence the occurrence of depression may be the degree of pain and the degree of education of patients.The development and prognosis of shoulder joint pain is closely related to mental and psychological factors.Clinical attention should be paid to the mental and psychological conditions of such patients.Active psychological counseling or appropriate drug treatments may have positive significance for somatic disease itself.
