Objective:This study aims at investigating the quality of life,psychological status and medication compliance of rheumatoid arthritis(RA)patients in Binzhou City,Shandong,China using a questionnaire survey.Based on emotional and drug dependence-related factors,we analyze the effect of a peer education model on the health education of these RA patients.Methods:The random sampling method was used.Between May 1,2016 and January 1,2017,160 patients with RA were selected as subjects from a 3-A hospital in Binzhou City.This study was organized and conducted via a randomized controlled trial.All patients met the 1987 ACR RA classification criteria.They were randomly divided into 2 groups:Control group and Intervention group.Eighty patients in the control group accepted traditional health education after discharge within three months,which was implemented by one nurse by telephone.The remaining eighty RA patients in the intervention group were randomly divided into 5 groups(15 in each group)and 5 companion educators who supplied peer education were randomly assigned to the 5 groups.After three months,quality of life scale(SF-36),Psychology Questionnaire(Zung's self-rating anxiety scale and Zung's Depression Self-Rating Questionnaire)and medication compliance(Morisky Medication Compliance)were given to evaluate the effect of health education on quality of life,negative emotions and medication compliance between these two groups.Data was analyzed and the difference was statistically significant,when P value is less than 0.05.Results:(1)For patients' backgrounds,there is no statistically significant difference between the control group and the intervention group.(2)The quality of life before treatment was not statistically different between the two groups.Significant difference was observed in quality of life evaluated by the 8 Dimensions score in the control group 3 months after discharge,which was not obvious in emotional function RE and the overall health GH.The 8 Dimensions score of the quality of life was significantly higher in the intervention group compared to that before the intervention.There was statistically significant difference between the groups' quality of life in eight dimensions of mental VT,social function SF,emotional function RE,and mental health MH score three months later after discharge.(3)The scores of SDS and SAS were not statistically different between the two groups before treatment.The SAS score was significantly higher than that before the intervention(P<0.05)in each group.The scores of SAS were statistically different between the two groups in 3 months after discharge(P<0.05).(4)There were no statistically significant differences in the compliance of medication between the two groups of patients.Whereas,differences were observed three months after discharge between the two groups,which are statistically significant.(5)Levels of anxiety,depression and life quality of the various dimensions of correlation analysis found that anxiety level and social function scores had a significantly negative correlation-the lower the level of anxiety in the patients with RA,social function,the better;on the other hand,the worse.The level of anxiety,depression and energy scores were significantly negative,indicating that the negative emotion level of RA patients was worse.There was a significant negative correlation between depression levels and mental health scores,suggesting that the higher the depression level,the worse the mental health.Conclusion:(1)The mean of anxiety of RA patients is slightly higher than the normal standard,indicating RA patients with predominantly negative emotions.(2)Peer education is more effective than a conventional telephone interview in improving the quality of life and medication compliance of RA patients(3)Peer education is more effective than conventional telephone interviews in improving the depression of RA patients...
